Reporting to the Social Work Manager, the Social Worker maintains a caseload of participants working in close coordination with the IDT. In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between participants, family members, support networks, and care teams as appropriate.

This role is different because Social Workers at WelbeHealth:

<li>Act as an integral member of an IDT rather than working on an island</li>
<li>Collaborate with LCSWs on complex behavioral health issues</li>
<li>Establish deep relationships with participants on caseload</li>
<li>Help participants age safely and independently in their homes with community resourcing</li>
<li>LCSW hours supervision available after 6 months of employment at no cost</li>

On the day-to-day, you will:
<ul>
<li>Conduct social work assessments to determine the psychosocial needs, preferences, and goals of participants, and actively participate in IDT meetings to develop participant care plans</li>
<li>Deliver and document social work interventions as agreed upon in participants' care plans, including but not limited to arranging necessary resources and services, assisting with care transitions, providing individual and group counseling, and case management</li>
<li>Partner with the Primary Care Physician and other IDT members to guide smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home)</li>
<li>Initiate, coordinate, and facilitate care conference meetings to ensure the highest level of care coordination among other care team members, participants, and people within the participants' support network (family, informal caregivers, etc.)</li>
<li>Provide discharge planning when participants disenroll from the program</li>
</ul>
Job requirements include:
<ul>
<li>Master's degree in social work (MSW) required</li>
<li>Minimum of one (1) year of experience working with a frail or elderly population preferred</li>
<li>Experience coordinating and facilitating care conference meetings preferred</li>
<li>Experience assisting people with behavioral health or substance abuse issues preferred</li>
<li>Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred</li>

Social Worker

The Medical Assistant focuses on assisting the medical staff (providers, nurses, and on-site specialty care providers) in the provision of safe and efficient delivery of medical services at the WelbeHealth clinic. The Medical Assistant performs routine administrative and clinical duties such as scheduling medical appointments, greeting participants, filing medical records, taking and recording vital signs, preparing exam rooms, and maintaining equipment and supplies.

This role is different because Medical Assistants at WelbeHealth:

<li>Engage in both clinical care and front-office interactions, fostering meaningful participant relationships while maintaining hands-on medical responsibilities, unlike traditional Medical Assistant positions focused solely on back-office or clinical work</li>
<li>Have the time and support to provide quality care and build trust, balancing efficiency with connection, rather than rushing through high patient volumes</li>

<li>Prepare participants for appointments, including but not limited to setting up exam rooms, checking participants in and out, and obtaining an accurate history of illness</li>
<li>Perform required pre-exam physical tests, such as measuring and documenting participants' vital signs</li>
<li>Perform general medical procedures, such as dressing changes and suture removals, as directed and under specific authorization and supervision of a physician</li>
<li>Collect and prepare laboratory specimens as ordered, and perform routine sterilization procedures of medical and dental equipment</li>
<li>Maintain clinic logs and ensure exam rooms are properly stocked, sanitized, and re-prepped at all times</li>
</ul>
Job requirements include:
<ul>
<li>Certification by a medical assistant certifying organization approved by the state medical board</li>
<li>Two (2) years of experience working as a Medical Assistant in a clinical setting with a frail or elderly population</li>
<li>Two (2)-year degree program in medical assistance</li>
<li>Phlebotomy experience required</li>
<li>Reliable means of transportation</li>
<li>Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred</li>
</ul>
