What You'll Do (Essential Job Functions)<ul><li>
Follow individual care plans and report any concerns to nursing or management.</li><li>
Administer medications and treatments with proper documentation, per policy.</li><li>
Support daily living needs, including:<ul><li>
Personal care
</li><li>Meal preparation and dietary support</li><li>
Household tasks and errands</li><li>
Health appointments and community integration</li><li>
Building natural supports and social relationships</li></ul></li><li>
Promote health, safety, and a clean home environment.</li><li>
Remain calm and effective during challenging situations.</li><li>
Foster a positive, respectful team dynamic with coworkers, individuals, and their families.</li><li>
Accurately document individual progress and changes.</li><li>
Attend all required trainings, meetings, and ongoing professional development.</li></ul>
What You'll Need (Minimum Requirements)
High school diploma or GED
Valid driver's license and reliable transportation
Ability to lift 25-50 lbs. (with assistance or lifting devices)
Physical ability to bend, squat, push/pull, and kneel as needed
A compassionate and patient mindset
