Under the direction of the MAT Administrative Manager and supervision of the Clinical Manager, and in conjunction with Alcohol &amp; Drug (A&amp;D) therapeutic and medical staff, the RECOVERY+ Case Manager is responsible for offering an array of recovery support and case management services to service recipients seeking treatment for substance use disorder(s) only or co-occurring disorders often in conjunction with medication-assisted treatment (MAT) as well as certain clients with trauma-specific circumstances. These responsibilities include but are not limited to assessments, case management and intensive case management services in person and via telehealth, and scheduling assistance. Responsibilities also include the coordination of care with clients and their systems of care across potential changes in housing or incarceration status, insurance and benefits changes, and any other impairment in activities of daily living that may be the result of a Substance Use or Co-Occurring Disorder or may impede the recipients&rsquo; progress in treatment for the disorder(s).
 
 
The RECOVERY+ Case Manager is responsible for providing, coordinating, and monitoring services for his or her respective caseload thirty-six (36) to forty (40) hours per week. This position is in partnership with BHG Savannah and Pathways.
<ul><li>Provides client and family education, support, and care in gender-, spiritually-, economically- and culturally-responsive, humble, kind, and competent ways
</li><li>Follows or exceeds HIPAA guidelines and maintains the highest level of confidentiality and privacy at all times when dealing with client information
</li><li>Provides services to people in recovery to promote individual, program and system-level approaches that foster health and resilience, increase permanent housing, employment, and other necessary supports while reducing gaps in service and barriers to social inclusion
</li><li>Facilitates equitable services for recipients in person and via telehealth as needed
</li><li>Facilitates smooth transitions from incarceration, residential treatment, or inpatient withdrawal management treatment to outpatient MAT services
</li><li>Develop and/or maintain a MAT-focused Treatment Plan related to recovery and co-occurring services that supports service recipient strengths, goals, and choice
</li><li>Develops, implements, reviews, and updates DLA-20 Assessments, ASIs, ASAMs, GPRAs, and Aftercare Plans per state, grant, insurance provider, agency, and program standards
</li><li>Provides and documents linkage, referral, and advocacy services to help the service recipient access and mobilize resources that meet his or her needs and encourage ongoing recovery from substance use and co-occurring (complex) disorders
</li><li>Completes all appropriate documentation in a timely manner with correct and appropriate tone, grammar, and syntax
</li><li>Collaborates effectively with service recipients, outside agency personnel, family members, and internal agency personnel including but not limited to supervisors, therapists, physicians, nurses, and case managers to ensure continuity of care
</li><li>Provides reporting of work activity to Clinical Manager and Administrative Manager as requested
</li><li>Participates in staffing and supervision as scheduled, and displays evidence of the value of lifelong learning as related to best practices in client care
</li><li>Participates in Quality Assurance &amp; Improvement, Utilization Management, and other agency and system functions
</li><li>Other activities or responsibilities as needed per supervisor or agency
</li></ul>JOB SPECIFICATIONS: 
EDUCATION:
<ul><li>Bachelor&rsquo;s degree required
</li></ul><ul><li>Bachelor&rsquo;s degree in social work, counseling, sociology, psychology or related behavioral-health field preferred
</li></ul><ul><li>Bachelor&rsquo;s degree in another discipline must be accompanied by fifteen (15) college-level semester hours of coursework in behavioral health or one (1) year of work experience in a behavioral health setting. 
</li></ul>LICENSURE, REGISTRATION, CERTIFICATION:
<ul><li>Must have a valid driver&rsquo;s license
</li></ul><ul><li>Must obtain and maintain certification in Non-Violent Crisis Intervention within the 90-day orientation period
</li></ul><ul><li>Any additional certification or training as requested by supervisor or Agency 
 </li></ul>EXPERIENCE:
<ul><li>Minimum of one (1) year of behavioral health care experience preferred
</li></ul><ul><li>Experience with clients diagnosed with substance use or co-occurring disorders preferred 
</li></ul>KNOWLEDGE, SKILLS AND ABILITIES:
<ul><li>Exhibits proficiency in Microsoft Word, Excel, Outlook, and PowerPoint
</li></ul><ul><li>Exhibits capacity to learn and utilize electronic medical records and other online software programs quickly and effectively

</li></ul><ul><li>Demonstrates ability to set priorities for tasks, to work effectively under minimal supervision, exceptionally organized, described by others as a self-starter, reliable, dependable, and motivated

</li></ul><ul><li>Able to use CPI training to effectively deescalate and safely restrain an individual if needed
</li></ul><ul><li>Able to work hours as designated by client need and provider availability when needed 
</li></ul>ENVIRONMENTAL CONDITIONS:
<ul><li>Prepared to travel throughout the agency&rsquo;s catchment area if needed, or where consumers reside to provide services including but not limited to locations such as other offices, criminal justice complexes, treatment centers, and client homes if needed

Job Description Summary:
Under the general supervision of the Director, Clinical Manager, and Charge Nurse. Responsible for providing hygienic care and assigned procedures for assigned patients based on age-specific needs.
<h2 class="jobSectionHeader">ESSENTIAL JOB FUNCTIONS:
</h2><div><div><div><ul><li>Answers call lights by carrying wireless phones or by the console.
</li><li>Obtains and records patient Vital Signs per unit specification.
</li><li>Assists with new admissions by taking vital signs, obtain weights, and assist patient with a gown, places telemetry patches, leads and telemetry box on patients.
</li><li>Responsible for repositioning patients every two hours, whether it is turning or encouraging patients to turn themselves.
</li><li>Ambulates patients in the hall as needed.
</li><li>Assists patients with meals by getting patients up to the chair before if appropriate offers hygiene and prepares the area for eating.
</li><li>Passes food trays and feeds patients who cannot feed themselves.
</li><li>Collects and accurately records intake and output.
</li><li>Assembles patient&rsquo;s charts, records patient information on unit records and routes patient&rsquo;s charges.
</li><li>Enters any written orders into the electronic medical record, completes related forms, schedules procedures via computer and/or manual process, adds forms related to procedures to chart and notifies licensed personnel.
</li><li>Assists with transport patients.
</li><li>Helps keep department tidy by putting up stock, and clean rooms such as break room, microwave, refrigerator, med room, dirty room, linen room, and equipment room.
</li><li>Assists with incentive spirometers.
</li><li>Responsible for documenting: baths, turns, Oral Intake and Output obtained from Foley catheter, urinal or collection vessel, shaves, mouth care, peri and or Foley care, Vital Signs, linen changes, shampoos, weights, the percentage of meals consumed on all of their assigned patients, ambulation, incentive spirometer usage, bowel movements, and the number of times patients were up in a chair.
