Other benefits include life, health, dental, and vision insurance, and a 401k with company match.Qualifications<ul><li>Bachelor&rsquo;s degree in social work (B.S.W) or related field required</li><li>Minimum of two (2) years of professional experience in aging services, intellectual and developmental disability (IDD) services, or a related human services field required</li><li>One year experience managing and supervising staff, required</li><li>Experience in budgeting and fiscal responsibility, preferred</li><li>Pass a BCI clearance</li><li>Valid driver&rsquo;s license and reliable transportation required</li><li>Must maintain appropriate auto insurance coverage</li></ul>Specific DutiesThe duties listed below are primary functions of the job, and are not intended to be all-inclusive.<ul><li>Develop a program work plan in conjunction with the Westbay work plan</li><li>Define roles and responsibilities of program staff</li><li>Train and supervise staff</li><li>Conduct performance appraisals of program staff</li><li>Oversee day to day operation of the case management programs</li><li>Monitor programs for compliance with federal, state, and local guidelines</li><li>Monitor client intake and certification eligibility to ensure compliance with established guidelines</li><li>Ensure all client referrals are assigned to staff and monitored</li><li>Monitor each caseload</li><li>Maintain accurate program, statistical, and client records and submit reports as required</li><li>Assist with the preparation of records for billing purposes</li><li>Supervise and train interns, monitoring their work</li><li>Provide case management services, including initial contact with the client.</li><li>Conduct a client assessment and development of a care plan</li><li>Provide follow-up services to senior clients in Home Care programs and those designated by OHA as abuse or protective clients</li><li>Conduct home visits as needed</li><li>Engage family and community support systems as available</li><li>Provide supportive counseling and advocacy to clients and their families</li><li>Coordinate services with other community resources</li><li>Determine eligibility needed resources and aid in the application process</li><li>Monitor clients and modify care plans as needed</li><li>Inform clients of their rights and responsibilities</li><li>Support client&rsquo;s efforts to have their voice heard and to get needed services.</li><li>Offer advocacy on the client&rsquo;s or family&rsquo;s behalf when needed services are not being adequately provided by an organization within the community&rsquo;s service delivery system</li><li>Recognize potential conflicts of interest and refer consumers to the next appropriate service to ensure advocacy needs are met</li><li>Provide information and referral services to telephone callers and clients of Westbay Community Action</li><li>Maintain information needed for data collection and enter into the agency's data system</li><li>Participate in meetings and trainings as needed and in public policy forums when appropriate</li><li>Educate the general community about senior services and Westbay</li></ul>Westbay Community Action, Inc. is an equal opportunity employer.
