Position Summary: The Transplant Coordinator is a specialized healthcare professional responsible for managing all aspects of care for patients undergoing organ or tissue transplantation. This includes coordinating evaluations, donor matching, surgical scheduling, and post-transplant follow-up. The coordinator acts as the primary liaison between patients, families, the transplant team, and external agencies to ensure safe, timely, and effective transplant care.Key Responsibilities:<ul><li>Patient Coordination:</li><li>Guide patients through the transplant process from referral to post-transplant follow-up.</li><li>Educate patients and families about transplant procedures, medications, and lifestyle modifications.</li><li>Assess patient readiness and ensure completion of all required evaluations.</li><li>Clinical Management:</li><li>Collaborate with multidisciplinary teams (surgeons, physicians, nurses, pharmacists, social workers, etc.) to develop and implement individualized care plans.</li><li>Monitor patient progress and coordinate pre- and post-operative care.</li><li>Participate in selection committee meetings and document patient eligibility decisions.</li><li>Donor Coordination:</li><li>Facilitate communication between donor programs, surgical teams, and recipients.</li><li>Verify donor-recipient compatibility, ensure regulatory compliance, and assist in organ procurement logistics.</li><li>Documentation &amp; Compliance:</li><li>Maintain accurate medical records in accordance with hospital policy and national regulatory standards (e.g., UNOS, OPTN).</li><li>Ensure timely reporting and data submission to transplant registries.</li><li>Education &amp; Quality Improvement:</li><li>Provide education to staff and the community about transplantation.</li><li>Participate in quality assurance activities and policy development for transplant services.</li></ul>Job Type: Full-timePay: &#8377;18,000.00 - &#8377;25,000.00 per monthWork Location: In person
