We are seeking a dedicated and compassionate Registered Nurse / LVN- Admissions to join our Hospice healthcare team. In this role, you will be responsible for conducting thorough patient assessments, coordinating admissions, and ensuring a seamless transition for patients into our care facilities. Your expertise will contribute to the overall quality of care and patient satisfaction.Duties<ul><li>Perform comprehensive patient assessments and evaluations during the admission process.</li><li>Collaborate with interdisciplinary teams to develop individualized care plans.</li><li>Document patient information accurately in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations.</li><li>Conduct triage assessments and prioritize patient needs effectively.</li><li>Administer medications and treatments as prescribed, including IV infusions and injections.</li><li>Provide education and support to patients and families regarding care plans and health management.</li><li>Maintain accurate medical records and documentation of patient care activities.</li><li>Participate in quality improvement initiatives and adhere to NCQA standards.</li><li>Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.</li></ul>Qualifications<ul><li>Current Registered Nurse (RN)/(LVN) license in the state of practice.</li><li>Experience in acute care settings such as hospitals or nursing homes is preferred.</li><li>Knowledge of medical terminology, anatomy, physiology, and critical care practices.</li><li>Experience with specific nursing skills including IV insertion, phlebotomy, suturing, catheterization, and airway management.</li><li>Background in geriatrics, hospice &amp; palliative medicine, or working with individuals with developmental disabilities is a plus.</li><li>Strong assessment skills and ability to manage acute pain effectively.</li><li>Excellent communication skills with the ability to work collaboratively within a team environment.</li></ul>Job Type: Full-timePay: $55.00 - $100.00 per hourWork Location: In person
