 The Bilingual Senior Counselor provides direct services/case management/coordination of care to an assigned caseload of individuals/families. This position assists in accessing needed supports and resources in the community, facilitates ongoing communication and advocacy between the family, DCF, school, probation and any other involved supports or service providers. This position may also facilitate/co-facilitate groups in accordance to standard practice.

ESSENTIAL DUTIES AND RESPONSIBILITIES
 Develops case management plans or plans of care and provides direct case management services and/or coordination of care specific to each assigned client and/or family (biological, foster and/or other caretakers as indicated), including interventions necessary to increase the probability of treatment completion, build on strengths of the client and/or family and to improve life functioning. Submits and reviews documentation as defined by program.
 Provides front line crisis support to individuals and/or families in the program by phone, in-home and/or in the community as needed based on initial assessment of the presenting concerns and program practice parameters. Problem solves by interpreting and adapting guidelines in the field and reviews situational details and assessment with supervisor for a second opinion and consultation.
 Makes decisions based on interpretation of the data presented and/or refines the methods and techniques used. Uses judgment to interpret and adapt program guidelines/procedures based on specific concerns/needs/problems presented by client/family.
 Determines the processes and methods to follow based on assessment of the client/family presentation and their strengths; takes a course of action from varying alternatives to provide client/family centered support and care.
 Provides group, individual and/or family counseling to clients with appropriate supervision and coordinates treatment phases with both internal and/or external service providers and/or family supports as appropriate.

 Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate.
 Provides service (evaluation, treatment, wraparound team facilitation, follow-up, case management) in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) and maintains a flexible schedule to ensure services are delivered based on client needs and as defined by the program.
 Provides/supports safety planning and crisis intervention/clinical emergencies with appropriate supervision including rotating on-call coverage with other team members to ensure 24 hour availability to clients as required by program.
 Participates in the development, implementation, and facilitation of initial treatment plan or plan of care, transition plan and timeline for strategies and next steps as defined by program.
 Demonstrates knowledge of internal and external levels of care, referral processes, and resources. Makes clinically appropriate referrals for identified support and service needs in a timely manner.
 Meets direct service productivity goals as defined by program, understands program/department/agency guidelines, goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.
 Identifies potential risk factors and takes measures to reduce those risks in the home through appropriate supervision, reinforcement of positive and safe behaviors, trauma-informed and relationally-based therapeutic interventions as defined by program.
 Attends and is prepared for scheduled supervision, team meetings, staff meetings or rounds to assist in coordinating case management/care coordination and review of treatment plans/plans of care. Seeks additional supervision or consultation as needed and follows through with supervisory directives.
 Communicates in a positive, effective manner with client, family, and all internal and external providers.

 The Recovery Support Specialist (RSS) is an individual who either is in direct sustained recovery, or who has been directly impacted by addiction. Using a motivational approach, the RSS will assist clients with linking to the appropriate recovery service based on identified need. In addition, the RSS will use an evidenced based model to provide ongoing support to clients who’ve received recovery services. Receive training and extensive supervision. Services may be delivered in community, office settings, and regional DCF offices.

ESSENTIAL DUTIES AND RESPONSIBILITIES
 Provide education on addiction recovery and facilitate linkages to these services.
 
Provide toxicology screens and SBIRT immediately upon referral during the agreed upon hours they are at each DCF Area Office. Toxicology screens will be conducted utilizing quick read cups or a breathalyzer. The recovery support specialist can also conduct hair sample screens, as requested by DCF.
 
Utilize the GAIN Short Screener
 (GAIN-SS) to screen for mental health, substance use disorder, and crime/violence.
 
Provide RMC for clients for up to six months following discharge from substance use treatment. RMC will include frequent check-ins in person (at clients’ homes, in the community, at DCF), over the phone, and by text. Initially the recovery support specialists will contact clients on a weekly basis to check-in. The frequency of contact will decrease as clients strengthen their recovery capital. Throughout all contacts, the recovery support specialists will provide mentoring and will serve as a role model providing hope that recovery is possible. Recovery support specialists will be available to provide initial RMC check-ins with new clients within one day of discharge from treatment. This will increase engagement and ensure that clients know they have a peer resource who is there to support them in achieving their recovery goals.
 
Proactively connect clients to local recovery resources and supports and will seek to identify concerns or relapse early and re-engage clients in treatment services, as needed.
 
Assist clients with ensuring they are following their child safety plans.
 
Provide recovery education to service recipients for every phase of recovery.
 
Assists in professional training, outreach, and public education services as assigned. This may include professional conferences, workshops and training for behavioral health agencies. May represent the agency at community forums or serve on inter-organizational committees as directed.
 
Documentation of clinical activities in client record as required by Wheeler Clinic.
 
Actively identify and support linkages to community resources (communities of recovery, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.) that support the recovering person’s goals and interests. This will involve a collaborative effort including the recovering person, agency staff and other relevant stakeholders.
 
Assist recovering persons to identify their personal interests, goals, strengths and weaknesses regarding recovery.
 
RSS will assume responsibility for coordinating the provision of services by other community professionals.
 
Refer to licensed substance use provider pursuant to the screen results and parent/caregiver agreement.
 
Provide reports on SBIRT findings to the parent/caregiver and DCF.
 
Parents/caregivers who test positive in a drug test and contest the results will be referred to a licensed substance use provider for a full evaluation.

Bilingual Recovery Support Specialist- SAFE Family Recovery

POSITION SUMMARY
 The Bilingual Integrated Health Care Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health and dental services.  The Bilingual MDFR (Multidimensional Family Recovery) Specialist will spend the majority of their time in the community meeting with clients in their homes or community locations where they feel comfortable. MDFR Specialists will provide MDFR in three phases with initial contact weekly, contact every other week during the second phase, and contact every three weeks during the third phase. Throughout, the goal is to help increase engagement in treatment services and foster stronger relationships and functioning within the family. The MDFR Specialist will focus on the caregivers as individuals, the children, parenting and co-parenting, parental intimate relationships, family relationships, and connection to the community. The specialist will provide brief MDFR interventions as identified by DCF and/or the family’s needs following the MDFR curriculum.
 
This position is eligible for a $4,000 sign on bonus! Half upon start and half after a year in the position.

On-site instant urine drug testing and/or breathalyzers that offer immediate results will be randomly performed per the model requirements. Drug testing for parents/caregivers who are in substance use treatment will be coordinated between providers to avoid duplication. Results are used to engage and sustain the parent/caregiver in their Recovery Support Plan.

 Actively participates in quality assurance activities including audio taping sessions and meeting with a quality assurance coach, attending trainings, and data report analysis and review.
 Participates in efforts to meet and maintain performance-based measures set by agency and CSSD.
 Participates in client-centered, strengths-based, presentations/discussions in staff meetings and attends teaming meetings with Probation, as required.
 Models pro-social behaviors and serves as a role model to clients and staff.
 Operates as part of a team by providing program coverage for client services (including urinalysis) outside of normal work schedule, typical job function or primary work site, as requested (e.g., additional evening shifts may be required to ensure operational coverage).
 Supervises toxicology testing. Supervised urine specimens shall only be collected from a client by a staff member of the same gender.
 Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
 Regulatory and Corporate Compliance Responsibilities
 Understands and adheres to Wheeler Clinic’s compliance standards as they appear in the clinic’s Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.
 Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HITECH, Infection Control, Sexual Harassment, etc.) and specific to the department and position.
 Performs other related duties as assigned.
