Job Summary</b></div><div></div><div><br>
The Nurse Supervisor ensures that exceptional patient care is met through the development and utilization of Nurses and Medication Trained Staff (MTS).</div>
Responsibilities:
<ul><li>Provides front-line supervision of Charge Nurses, Nurses, Day Program Nurses, and MTS on each shift.</li><li>
Covers nursing assignments and Charge Nurse Coverage as necessary.</li><li>
Reviews nurses&rsquo; notes to ensure that they are informative, descriptive of the nursing care provided, and reflect the resident&rsquo;s response to care.</li><li>
Monitors medication passes and treatments to assure that medications are administered as ordered and treatments are provided as scheduled.</li><li>
Makes rounds at residences and day programs on a periodic basis, providing supervision and support for Nurses, MTS MCS.</li><li>
Completes Quality Assurance checks with documentation.</li><li>
Listens to concerns of Nurses, MTS, and MCS regarding issues and reports concerns to the Nursing Manager with recommendations for resolution.</li><li>
Assists with obtaining any necessary supplies and equipment for nursing services by submitting paperwork through proper channels.</li><li>
Participates in training Nurses and MTS&rsquo;s. Provides in-service for Nurses, MTS&rsquo;s and other employees.</li><li>
In coordination with Nursing Manager, performs interviews, evaluations, and disciplinary action.</li><li>
Schedules Nurses and MTS&rsquo;s to provide appropriate coverage on each shift within budgetary guidelines.</li><li>
Assists Nurse Manager with monitoring and reconciling Kronos Workforce Central daily/weekly and processing appropriate paperwork to payroll.</li><li>
Assists Nurse Manager with reconciling employee mileage with Kronos Workforce Central monthly and processing paperwork to Accounting.</li><li>
Assists Nurse Manager with completing the initial Enterprise Incident Management (EIM) reports per 3800 and 6400 regulations and processes paperwork to Woods Quality Improvement department and appropriate agencies; also assists Nurse Manager with monitoring and completing the final 30-day reports when due.</li><li>
Participates in regular meetings with the Director of Nursing, Nursing Managers, Nurses, and MTS&rsquo;s.</li><li>
Participates in surveys (inspections) by authorized governing agencies.</li><li>
Provides clinical assessment of individuals as necessary. Communicates with the physician on-call, as indicated.</li><li>
Communicates, or assures communication, with families regarding health concerns.</li><li>
Reports to other disciplines regarding: individual&rsquo;s health issues, problems observed in units, or other concerns as necessary.</li><li>
Provides input into policies and procedures.</li><li>
Oversees` On Job Training with new hires(OJT)</li><li>
Has knowledge of and demonstrates computer proficiency.</li><li>
Serves as on-call Nursing Administrator, as scheduled.</li><li>
Maintains effective communication and working relationships with Woods&rsquo; administration and subordinates.</li></ul>
Qualifications:
<ul><li>Current licensure as RN in Pennsylvania.</li><li>
Valid driver&rsquo;s license and reliable transportation.</li><li>
Current CPR.</li><li>
Must be able to lift 50 pounds.</li><li>
Three years&rsquo; experience in the field of MH/MR.</li><li>

Job Summary</b></div><div>
Provides skilled nursing care to the individuals at day program. Acts as a liaison between Nursing Department and day program staff/management, while overseeing all health related day programing medical concerns.</div>
Responsibilities:
<ul><li>Provides skilled nursing care to individuals in the day program, including but not limited to assessment of acute and chronic concerns and medication and treatment administration.</li><li>
Provides in-services on health issues to day program personnel.
</li><li>Completes and maintains documentation on individual care services and keeps confidential medical information secure.</li><li>
Communicates to Nursing Dept., appropriate day program personnel including teachers and administration of any acute and emergent health issues that arise with individuals enrolled at the day program.</li><li>
Communicates with Clinical Charge Nurse any emergency issues immediately and all health issues by the end of their working shift.
</li><li>Contacts the guardians of individuals enrolled at the day program if any acute or emergent health issues arise.
</li><li>Demonstrates computer proficiency.<br>
</li><li>
Attends meetings for individuals as requested that occur in day programs, coordinating input with the professional disciplines at Woods.
</li><li>Collaborates with guardians, Medical Providers, and multidisciplinary professional staff to implement individualized care for those enrolled at the day program.
</li><li>Collaborates with Medication Trained Staff that are assigned to the day programs, orienting and educating them on specific medical treatments, changes in medications, new medications, etc.
</li><li>Maintains a safe and clean working environment, managing organization of office, medical records, and nursing supply inventory. Anticipates medical supplies needed and orders as necessary.
</li><li>Ensures operation of medical equipment by verifying emergency equipment availability, troubleshooting malfunctions, requesting repairs, and maintaining equipment inventories on a routine basis.</li><li>
Participates in any regulatory inspections with preparation and monitoring of health related documentation.</li><li>
Performs all other duties as assigned.
</li></ul>Qualifications:
<ul><li>Current licensure as RN/LPN in PA</li><li>
Two years&rsquo; experience in the field of Developmental Disabilities preferred</li><li>
Current American Heart Association issued Basic Life Support or Advanced Cardiovascular Life Support certification.

Job Summary</b></div><div></div><div><br>
The LPN provides normalized, therapeutic and preventative nursing care to individuals in a given assignment at Woods Services within the scope of the Pennsylvania Practical Nurse Law and in accordance with licensing standards and Woods Services Policies and Procedures. Maintains communication with Campus Nurse and Nursing Management.</div>
Responsibilities:
<div>
Evaluation</div><ul><li>
Evaluates individual&rsquo;s health status utilizing technical skills and knowledge on admission, when health needs indicate and on discharge. Performs more detailed assessments in conjunction with campus nurse as indicated.</li><li>
Requests back-up from campus nurse or another nursing staff when individual&rsquo;s signs and symptoms indicate the need.</li><li>
Performs monthly nutritional evaluation of individuals by weight and knowledge of dietary habits.</li><li>
Assists physicians with rounds in unit and clinics as necessary.</li><li>
Attends psychotropic medication reviews and actively participates in the multi-disciplinary treatment process.</li></ul><div>
Medication Administration</div><ul><li>
Prepares and administers medications as ordered by Woods&rsquo; physicians in accordance with Woods&rsquo; Medication Procedures.</li><li>
Reports therapeutic effectiveness of medication with pertinent data and any observed side effects or relevant laboratory values to Woods&rsquo; physicians.</li><li>
Reviews monthly medication sheets and quarterly physician order sheets from pharmacy and makes corrections as necessary, communicating with pharmacy.</li><li>
Administers approved immunizations as required after demonstrating adequate skills, knowledge of procedures, and immunizations.</li><li>
Documents all relevant information according to Woods&rsquo; Medication Procedure Manual on MAR sheets, physicians&rsquo; orders and nurses&rsquo; notes.</li><li>
Assists individuals with the self-administration of medications goals as indicated by the SAM assessment and IHP goals</li><li>
Accurately administers and documents treatments as ordered by Woods&rsquo; physicians. .</li><li>
Instructs, supervises, and insures documentation of any treatments in which direct care staff or MTS staff may be involved.</li></ul><div>
Communication Responsibilities</div><ul><li>
Documents evaluations in the medical chart and on all appropriate reports, forms, graphs, etc.</li><li>
Documents the health needs of specific individuals in residential logs on a daily basis, where applicable.</li><li>
Documents quarterly summary of health status including an update of all health issues and effectiveness of medications.</li><li>
Documents weekly on &ldquo;Medical Profile DYFS&rdquo; form for appropriate individuals.</li><li>
Prepares paperwork for annual physical examinations, GYN, neurological reviews, IHP&rsquo;s consults on and off campus, and completes hospital and emergency room transfer sheets.</li><li>
Completes division of paperwork forms and is responsible for same.</li><li>
Relays pertinent health concerns requiring follow-up (i.e. seizures, elevated temperatures, unusual behavior, rashes) to the campus nurse as necessary and on a daily basis at the end of the shift.</li><li>
Coordinates appointments with Health Services for individuals.</li><li>
Participates in Interdisciplinary Team process, as necessary, functioning as an integral member of the team. Documents monthly progress on identified goals.</li><li>
Coordinates lab work.</li><li>
Notifies family/guardians/agencies/Residential Manager/Day Program Manager of health issues as necessary. Contacts families/guardians/agencies for non-routine testing, consultations after clarifying insurance/funding.</li></ul><div>
Continuing Education</div><ul><li>
Participates in continuing education relevant to the area of nursing. Obtains minimum of 40 hour inservice training per year. Obtains CPR certification yearly, First Aid certification every three years, and other mandatory inservices.</li><li>
Conducts inservices as a resource person regarding health issues for other staff persons.</li><li>
Provides health teaching to individuals as necessary.</li><li>
Attends nurses&rsquo; meetings/inservices and serves on committees when requested.</li></ul><div>
Other Responsibilities</div><ul><li>
Covers units other than scheduled assignment when necessary for campus wide coverage. Rotates coverage of school programs. Carries beeper/radio for school and back-up coverage.</li><li>
Assists with emergencies.</li><li>
Maintains medical equipment in units, assesses functioning of emergency equipment monthly and after use, and reports any malfunctioning equipment immediately.</li><li>
Assists in orientation of new nursing employees.</li><li>
Performs other duties as assigned.</li></ul>
Qualifications:
<ul><li>Current licensure as Practical Nurse in PA.</li><li>
Current driver&rsquo;s license.</li><li>
Current CPR certification or completion of course within six months</li><li>
Must be able to lift/transfer a minimum of 50 pounds.</li><li>
One years&rsquo; experience in direct nursing care.</li><li>
Experience with Developmental Disabilities/MR preferred</li></ul><div><b>
