Advanced Practice Clinician
Nurse Practitioner / Physician Assistant
Nasson Health Care is seeking a qualified Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to work collaboratively with a team of health and administrative professionals to provide comprehensive primary care to patients while utilizing the Patient-Centered Medical Home (PCMH) model of care delivery.
Primary Responsibilities
The Advanced Practice Clinician:<ul><li>
Provides direct clinical services in accordance with the highest standards of medical and nursing practice and in full accordance with health center policies and protocols.</li><li>
Assures access for continuity of care through appropriate referrals for sub-specialty and inpatient services and prompt follow‑up of patients discharged from inpatient settings.</li><li>
Engages population health strategies to identify and address health needs among various segments of the primary care patient panel and makes optimal use of the electronic medical record system.</li><li>
Collaborates with other members of the practice team to prepare for patients&rsquo; office visits and assure that all indicated services are planned and delivered.</li><li>
Collaborates with other members of the practice team to manage the care of patients with high‑risk and complex conditions.</li><li>
Actively engages in ongoing provider panel management activities; routinely scans patient records for gaps in care and takes steps to assure that appointments are scheduled and all indicated services are delivered.</li><li>
Fully embraces and carries out all applicable NCQA Patient‑Centered Medical Home standards and Federally Qualified Health Center (FQHC) requirements.</li></ul> 
Qualifications
Nurse Practitioner
Applicants must have:<ul><li>
A Master&rsquo;s degree in Nursing</li><li>
An unrestricted Maine license to practice as an adult, family, or pediatric Nurse Practitioner</li><li>
A U.S. Drug Enforcement Agency (DEA) license</li></ul>
In addition:
A minimum of one to two (1-2) years of experience in a community health or primary care practice is required. 
Physician Assistant
Applicants must have:<ul><li>
A Master of Science in Physician Assistant Studies</li><li>
An unrestricted Maine license to practice as an adult, family, or pediatric Physician Assistant</li><li>
A U.S. Drug Enforcement Agency (DEA) license</li><li>
Compliance with NCCPA standards</li></ul>
A minimum of one to two (1-2) years of experience in a community health or primary care practice is required. 
Required Knowledge, Skills, and Abilities (Applicable to Both Roles)<ul><li>
Working knowledge of the core concepts of:<ul><li>
Evidence‑based practice</li><li>
Social and behavioral determinants of health</li><li>
Population‑based care</li><li>
Integration of medical, behavioral health, and dental care</li><li>
Working within an electronic medical record</li></ul></li><li>
Demonstrated respect for cultural diversity in serving patients and families.</li><li>
Ability to apply critical thinking skills in performing patient assessment and care.</li><li>
Willingness to work some non‑traditional hours.</li><li>
Ability to travel between YCCAC/Nasson Health Care clinical sites.</li></ul>

Medical Assistant
 
Full Time; Nonexempt
Career Ladder Progression Program Level 1 &ndash; Level 4
The Medical Assistant works collaboratively with a team of health professionals to provide comprehensive primary care to all patients while utilizing the Patient-Centered Medical Home model of care delivery.
The Medical Assistant:<ul><li>
Collaborates with other members of the practice team to prepare for patients&rsquo; office visits and assure that all indicated services are planned and delivered.</li><li>
Supports the attainment of optimal, patient-centered outcomes defined by a care planning process between providers, patients, and patients&rsquo; support members.</li><li>
Carries out new patient orientation session and prepares patients for provider visits.</li><li>
Assists with examinations, procedures and treatments.</li><li>
Performs in-office diagnostics, treatments according to practice guidelines and a variety of routine blood drawing procedures.</li><li>
Processes lab specimens and prepares and administers medications and immunizations.</li></ul>
 Qualifications:<ul><li>
Applicants must have Associate Degree in Medical Assisting or diploma from an Approved Practical Nursing Program.</li><li>
Experience in a primary care medical practice preferred.</li><li>
Capable of applying critical thinking skills in performing patient assessment and care.</li><li>
Demonstrated proficiency in using the electronic medical record and practice management systems.
</li><li>Ability to work as part of a team that supports integration of medical, behavioral health and dental care.</li><li>
Must show proof of immunity or have been immunized for MMR, Hep B (offered with employment), Influenza (required annually) and the COVID primary series.</li></ul>
