As a Physical Therapist here at Your Health, you will obtain complete health and psychosocial history and records findings in a systematic format. Perform complete physical assessment, while providing health education and counseling. Arrange referrals to other members of the health team as needed and to community resources. Physical Therapist must use appropriate reporting mechanisms as required by law, respect the dignity, confidentiality, and privacy of patients, obtain appropriate authorization and consents for treatments, review patient&rsquo;s medical diagnosis, prognosis, procedures, and clinical course.  Qualifications

As a Physical Therapy Assistant, you will work closely with licensed physical therapists to provide rehabilitative services to patients suffering from physical injuries or disabilities. Your primary responsibilities will include assisting in the implementation of treatment plans, conducting exercises with patients, educating patients on proper techniques for rehabilitation exercises, and documenting patient progress. As well as developing individualized treatment plans, instruct and assist patients in performing therapeutic exercises and activities, monitor patients during therapy sessions, providing guidance and support as needed. Maintain a clean and organized therapy environment, ensuring that equipment is properly sanitized and maintained. Communicate regularly with physical therapists and other healthcare professionals to coordinate patient care and adjust treatment plans as needed.
