What Are We Looking For?
Your Health is currently looking for a Respiratory Therapist t to join our growing primary care family. As a RT, you will work closely with licensed physical therapists to provide rehabilitative services to patients suffering from physical injuries or disabilities. Your primary responsibilities will include assisting in the implementation of treatment plans, conducting exercises with patients, educating patients on proper techniques for rehabilitation exercises, and documenting patient progress. As well as developing individualized treatment plans, instruct and assist patients in performing therapeutic exercises and activities, monitor patients during therapy sessions, providing guidance and support as needed. Maintain a clean and organized therapy environment, ensuring that equipment is properly sanitized and maintained. Communicate regularly with physical therapists and other healthcare professionals to coordinate patient care and adjust treatment plans as needed. Assist in the implementation of discharge plans, including providing patients with instructions for continued care and recommending appropriate follow-up services.
Qualifications

The Community Health Worker (CHW) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services. assembly and functionality. Provide training to patients and caregivers on the correct use of 
DME.</li>
<li>Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.</li>
<li>Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.</li>

Qualifications:
<ul><li>Certified Medical Assistant, Paramedic, or Licensed Nurse preferred. Certifications or licenses must be in good standing.</li>
<li>Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.</li>
<li>Previous experience in public health or home health preferred.</li>

<li>Interact with patients via email, patient portal, telephone, text, in-person, etc. to gather necessary information, schedule and confirm appointments in advance, and provide pre-appointment instructions.</li>
<li>Communication methods such as phone, email, AthenaText, Microsoft Teams, etc. will be utilized.</li>
<li>Appropriately and accurately document and log care management activities.</li>
</ul>
Qualifications
<ul><li>Must be a Certified Medical Assistant (CMA) or licensed nurse (i.e. LPN or RN). Must be licensed in the state of employment. Certification/license must be in good standing with the appropriate board.</li>
<li>A minimum of one (1) year experience in a healthcare setting preferred.</li>
<li>Must hold and maintain current CPR certificate.</li>
<li>High school diploma or equivalent required.</li>
