<ul><li>Develop departmental strategic objectives, KPIs, and individual employee goals in alignment with leadership direction.
</li><li>Ensure achievement of departmental targets through effective planning, risk management, and data-driven decision-making.
</li><li>Develop, manage, and monitor the annual departmental budget, ensuring alignment with financial plans and minimizing variances.
</li></ul>Core Activities Population Health Program Ownership
<ul><li>Lead the design and management of PHM programs, including screening, chronic disease management, and employee health initiatives.
</li><li>Define program scope, eligibility criteria, care pathways, care gaps, and outcome measures in collaboration with clinical specialties.
</li><li>Conduct disease segmentation and risk stratification to identify and prioritize high-risk populations.
</li></ul>Clinical &amp; Cross-Functional Collaboration
<ul><li>Partner with clinical specialties to integrate PHM programs into clinical workflows and ensure clinical alignment.
</li><li>Act as the PHM representative in clinical committees and leadership forums.
</li><li>Collaborate with ADA, Case Management, IT, and Quality teams to develop dashboards, KPIs, workflows, and care-gap resolution processes.
</li></ul>Value-Based Care &amp; Performance Management
<ul><li>Lead PHM contribution to value-based care initiatives, including pilot design and risk-stratification frameworks.
</li><li>Define PHM KPIs and ensure alignment with measurement logic developed with analytics teams.
</li><li>Interpret program performance data and contribute to executive and board-level reporting.
</li></ul>Patient-Reported Outcomes &amp; Care Delivery
<ul><li>Lead PROMs initiatives across PHM programs, ensuring integration with quality, patient experience, and IT teams.
</li><li>Support patient-centered care delivery, ensuring dignity, compassion, and shared decision-making.
</li><li>Promote continuous improvement through patient feedback and outcome tracking.
</li></ul>Qualifications &amp; Experience
<ul><li>Bachelor&rsquo;s degree in Nursing, Pharmacy, Allied Health, Physiotherapy, Public Health, or related clinical field (required).
</li><li>Master&rsquo;s degree in Public Health, Health Administration, Epidemiology, Population Health, or related field (preferred). MD is an advantage.
</li><li>6&ndash;8 years of relevant experience, including 2&ndash;4 years in a managerial role.</li></ul></div>

<div><p>DUTIES AND RESPONSIBILITIES<br>
</p><p></p><ul><li>Provide personalized coordinated care, support and treatment for patients and families.
</li><li>Treat people with dignity, compassion, and respect.
</li><li>Involve patients, families and caregivers by providing them with education and resources for offering optimal support to promote recovery.
</li><li>Acts as a general receptionist for the unit with a welcoming manner and helpful attitude.
</li><li>Performs clerical duties related to generating monthly statistics, policies and procedures.
</li><li>Provides clerical support to the department including filing of test results, maintaining stock of forms and charting supplies.
</li><li>Processes physician orders as per policy including daily workloads.
</li><li>Assists the Staff by periodically checking and subsequently Discharging Patients awaiting Department Clearance.
</li><li>Utilizes the communication systems in an efficient and effective manner (telephone, computer, pneumatic tube and mail system etc.)
</li><li>Acts as part of the team by ensuring communication is passed on to the appropriate person in a timely manner, maintaining department logs and correspondence files.
</li><li>Communicates clearly and effectively with other departments in booking procedures, tracking information and obtaining supplies or equipment for staff and patients.
</li><li>Assists the staff with data processing needs.
</li><li>Performs duties in a timely manner and one that respects the confidentiality of patient information.
</li><li>Assist with the orientation of new staff as it relates to the clerical functions.
</li><li>Performs other tasks and duties within the scope of the knowledge, skills, and experience required for the job, as assigned by the immediate supervisor.
</li><li>Ensures store are neat and tidy .all consumable bins are properly labeled
</li><li>Performs periodic audit in the store
</li><li>Utilizing the communication system in efficient and effective
</li></ul><p>QUALIFICATIONS :
</p><ul><li>Graduation from high school
</li><li>Preferably fluent in English
</li><li>Post High school clerical course, preferable
</li></ul><p>PROFESSIONAL EXPERIENCE:
</p><ul><li>Post high school clerical course, preferable with medical terminology or appropriate experience as determined by the Human Resources Department.
</li></ul><p>SPECIALIZED SKILLS:
</p><ul><li>Ability to read and communicate effectively in English.
</li><li>Good computer knowledge
</li><li>Typing/keyboard at 40 words per minute.
</li><li>Work may require lifting or pushing in excess of 30 pounds.
</li><li>Work requires walking, running, climbing and bending</li></ul></div>
