1099 Medical Doctor, IPV (20+ state licenses required)

In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services.

• Responsible for establishing the Tapestry Health patient relationship for our chronic care management services.

• Be available at a minimum of 8-12 working days per month to conduct approximately 300-375 clinical visits onsite.

• Document patient encounters accurately and thoroughly within our platform at time of visit.

• Provide end of day report to program administrators to track progress for completion of visits.
  
  

• Qualified MD/DO

• Active, unrestricted, Medical Licenses in multiple states

• Excellent clinical judgment and decision-making skills.

• Must have reliable transportation to travel to assigned buildings.

• Commitment to providing patient centered care and improving health outcomes.

• Familiarity with electronic health record (EHR) systems and healthcare technology.

• Ability to travel within 1-2 hours of assigned region
