Community Counseling Center of Ashtabula County is a non-profit behavioral health provider focused on engaging the community in recovery.
CCC has an immediate, full-time position available for a Access Care Coordinator whose responsibilities include providing, coordinating, linking and scheduling services to the severely mentally disabled and/or chemically dependent adults, children and families in office-based settings, and ensuring appointments scheduled coincide with client’s insurance requirements, client location and provider preferences and assisting client to utilize technology to complete required documentation.
This position is eligible for a signing bonus of up to $1000.

ACCESS CARE COORDINATOR position details are:
EDUCATION REQUIREMENTS: High School diploma or equivalent required.
LICENSURE/CERTIFICATION REQUIREMENTS: LSW/LPC preferred; The position requires: a valid driver’s license with limited driving infractions within the last three (3) years subject to the Agency Employee Handbook/Policy; no DUI/OVI, drug, theft, or felony convictions in the past, except as allowed under the provisions and exclusionary period outlined in Ohio Administrative Code, Rule 5160-1-17.8; the provision of reliable transportation; adequate levels of insurance and insurability.
EXPERIENCE REQUIREMENTS: Two to five years’ experience working with severely, persistently mentally ill, severely emotionally disabled and/or chemically dependent population preferred. One year experience with electronic medical health record, spreadsheet and other business applications preferred but will train selected candidate.
KNOWLEDGE & SKILLS REQUIREMENTS: The position requires the ability to communicate effectively verbally and in writing over different types communication modes; the ability to establish and maintain effective relationships with clients, significant others, general public and stakeholders; the ability to function as part of an organization by working with others and by adhering to established policies and procedures; and the ability to assemble meaningful information in order to draw valid conclusions. The position requires ability to work independently as well as cooperatively, reliably and dependably, and the ability to manage available resources to carry out assigned tasks. This position requires the ability to manage multiple tasks simultaneously and the ability to problem solve for resource-related and severe/acute mental health needs. It requires the ability to learn, understand, and utilize different databases related to client charts, insurance providers, and referral scheduling. It requires the ability to provide services via telehealth and in-person in accordance to client preference. This position requires training in First Aid, CPR and De-escalation techniques.
WORKING SCHEDULE: The Agency is open 8:00 am – 7:00 pm, Monday through Thursday and 8:00am – 5:30 pm on Friday. Satellite office hours vary but will typically be open between 8:00 am – 5:00 pm, Monday through Friday. Hours for this full-time position are dictated by client and Agency need at a minimum of a 40-hour workweek.
DESCRIPTION: Responsible to complete outreach functions to clients indicating need for service, support required grant tracking functions by providing follow-up phone surveys with clients, and perform other duties to support client entry in obtaining agency services. This may include assuring provision of necessary services to resolve or minimize the effects of a client's impairment including assessment of level of impairment, crisis linkage and coordinating mental health care with clients for inpatient and outpatient mental health needs, ensuring clients have access to/knowledge of their rights as clients and providing referrals for basic needs during initial and ongoing care coordination appointments. This position will have additional job duties that cover Pharmacologic Management (PHM) support, Telehealth coordination between provider and client, Urine Drug Screening management and location-specific stakeholder support dictated by a Memorandum of Understanding.
