EMPLOYMENT TYPE: Full-time, 40 hours/wk

SCHEDULED HOURS: Monday-Friday, 8AM-4:30PM

ABOUT THE PROGRAM: Triage Center

Responds to statewide requests for CHR services. Provides clinical screening & triage to multiple levels of care. Refers individuals and/or families to the appropriate level of care in or out of our service system. Crisis triage to 911/emergency services and/or MCRT & EMPS (via 211) as needed. Triage Center is staffed by a team of call center agents, a care coordinator, a clinical support staff and a manager.

ABOUT THE POSITION: Access Coordinator

Duties & Responsibilities

  • Manages the flow of Open Access patients across five clinic sites, by assigning clinicians, scheduling appointments and ensuring intake paperwork is completed.
  • Establishes rapport and working relationships with clients, parents, other support staff, and clinical staff in order to engage clients in treatment
  • Review client charts to determine eligibility for services based on CHR policy guidelines and physician recommendations.
  • Provide education to clients about resources to assist them in accessing treatment, i.e., transportation alternatives, schedules of providers
  • Coordinate with other CHR staff to schedule hospital, urgent care, and routine intakes for consumers seeking treatment.
  • Document and track the daily workflow of intakes and other Open Access activities
  • Assist clients to complete intake paperwork and assessment tools needed to appropriately evaluate their needs
  • Work with Clinical Program Directors, Supervisors, and Practice Managers to quickly assign patients to appropriate clinician for an intake.
  • Coordinate logistics and schedule the telehealth session for each individual appointment coordinated.
  • Assist in follow up when consumer is referred elsewhere after assessment
  • Meets all recordkeeping requirements of program and agency standards including monitoring and charting all outreach and engagement attempts.
  • Attends and participates, as appropriate, in staff meetings, case conferences, and supervision.
  • Works with Triage Center, Engagement Specialists, and front desk staff to coordinate access for clients who are being discharged from higher levels of care, or having difficulty connecting to outpatient level of care.
  • Provides excellent customer service to all clients and families.
  • Assist staff in promoting a recovery-oriented treatment environment.
  • Stays abreast of all agency and clinic policies regarding access, no shows and scheduling.
  • May perform other assignments and duties as program and agency needs require.

QUALIFICATIONS:

Education: Associates Degree in a MH or SA field preferred, or high school diploma with three years’ experience in the mental health or SA field.

Experience: Experience in working in fast paced environment with consumers of MH and SA services.

COMPENSATION*:

$19.285/hr and up, dependent upon experience

  • Actual rates are determined at the time of offer and are based on relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
