ACT Case Manager

This opportunity is a uniquely rewarding and challenging position for an individual interested in working with a team of professionals/paraprofessionals to provide comprehensive behavioral health services to high risk consumers in the community.

POSITION SUMMARY:

• Participating in daily staff meetings.
• Providing case management services
• Working with a team to ensure that basic needs of the consumers are met.
• Completing face-to face home visits.
• Participating in an on-call rotation.

POSITION REQUIREMENTS:

• Bachelor’s Degree in a Human Service-Related Field from a regionally accredited college
• Must Possess Valid WV Driver’s License
• Access to a Reliable Vehicle
• Applicant must be able to clear all Background Checks

Physical Requirements

1. Frequent communications, verbal and written.

2. Frequent walking (including stairs) standing and/or sitting.

3. Visually or otherwise identify, observe and assess consumers.

4. Ability to perform verbal and nonviolent physical crisis intervention techniques & CPR.

Positions are full time with the typical work week being Monday through Friday, 8am – 5pm and participate in a rotating on-call system.
