ACT Team Registered Nurse/Team Leader

HHCI is in search of a Registered Nurse (RN)/Team Leader (TL) for its Assertive Community Treatment (ACT) Team. This position is field-based, and will require the RN to report to the office (located in Baltimore, MD) as needed.

Position Summary:

The ACT team RN provides ACT services to a mutually shared caseload of ACT clients within HHCI’s treatment and recovery services division. The RN is responsible for utilizing evidence-based practices to assist their clients with meeting their treatment and recovery goals, and operating within the scope of their profession.

The RN also provides clinical and administrative supervision of all functions of the ACT program within the Treatment and Recovery Services division, which is a multi-disciplinary program. The RN ensures that the program is operating in compliance with all of HHCI’s regulators and governing bodies, clients are receiving high quality care, and personnel are utilizing evidence-based practices to assist clients with achieving their treatment and recovery goals.

Responsibilities:

  • Provide direct client treatment and care to each admitted client of the ACT team at least monthly
  • Facilitate daily team meetings
  • Conduct intake screenings for prospective clients
  • Complete and submit ACT tracking spreadsheets in collaboration with the ACT Program Assistant
  • Review, approve, and address any discrepancies with clinical documentation of direct reports, including progress notes and assessments
  • Provide clinical and administrative supervision to direct reports
  • Conduct nursing assessments within 30 days of admission for newly admitted clients, and every six months thereafter for existing clients
  • Coordinate referrals for medical/health services needed for admitted clients, including referrals to primary care physicians and specialists
  • Request, track, monitor, and measure lab results from admitted clients bi-annually
  • Administer long acting injectable medications prescribed by the ACT psychiatrist/nurse practitioner
  • Update and maintain a long acting injectable medication log
  • Accompany clients to appointments, if needed
  • Enter, sign, and submit all electronic clinical documentation, including progress notes and assessments, within the deadlines established by the program
  • Participate in interviews with candidates for vacant positions
  • Onboard and provide clinical training to new hires, and train existing personnel
  • Participate in collaborative meetings with human resources regarding hiring needs
  • Communicate and collaborate with other divisions/departments within the organization
  • Participate in at least two internal committees as determined by the CEO
  • Conduct chart audits as assigned by the Quality and Compliance department and CD’s direct supervisor, as well as conduct unassigned chart audits
  • Provide client coverage in the absence of direct reports
  • On-call responsibilities via phone as needed
  • Collaborate with consented external providers and internal personnel regarding the treatment goals of mutually shared clients
  • Complete required trainings within established timeframes
  • Review, approve, and address any discrepancies with timecards, and time off requests
  • Complete personnel performance reviews
  • Participate in the performance improvement/progressive disciplinary process
  • Comply with all relevant standards and regulations of HHCI, the state of Maryland’s Behavioral Health Administration (BHA), The Joint Commission, HIPAA
  • Perform other duties as requested by supervisors

Required Qualifications

  • Bachelor of Science in Nursing degree from an accredited BSN program
  • Licensed in the state of Maryland as a Registered Nurse, or APRN/PMH
  • A minimum of 3 years of supporting the mental/behavioral health population post-licensure as a RN
  • A minimum of 1 year of experience supervising mental/behavioral health professionals from multiple disciplines within a mental/behavioral health treatment program
  • Proficiency in Microsoft Suite (Word, Excel, Outlook, Power Point, Access), electronic health records, and the use of computers/laptops

Preferred Qualifications:

Strong preference for experience -

  • Supervising/collaborating with mental/behavioral health professionals from multiple disciplines
  • Supporting individuals who are diagnosed with serious and persistent mental illness, and/or dually diagnosed with mental health and substance use conditions
  • Working with a multi-disciplinary mobile team

Job Type: Part-time

Pay: $49.00 - $54.00 per hour
