ACT Vocational Rehabilitation Case Manager

Valley Community Services Board is seeking a dynamic individual to join our Assertive Community Treatment Team. The ACT Vocational Rehabilitation Case Manager is a key member of a multidisciplinary Assertive Community Treatment (ACT) team, providing employment-focused services to individuals with serious mental illness. This role supports clients in obtaining and maintaining meaningful employment through individualized, recovery-oriented, and community-based interventions.

Summary of Essential Functions

  • Provide vocational assessment to identify client strengths, interests, and employment goals.
  • Deliver supported employment services using evidence-based practices (e.g., IPS model).
  • Assist clients with job search activities, including applications, resumes, and interview preparation.
  • Develop relationships with local employers to create job opportunities for clients.
  • Provide on-the-job support and coaching to promote job retention.
  • Collaborate with ACT team members to integrate vocational goals into overall treatment planning.
  • Maintain accurate and timely documentation of services in accordance with agency and regulatory standards.
  • Educate clients on workplace expectations, benefits planning, and disclosure decisions.
  • Provide transportation or accompany clients to employment-related activities as needed.
  • Monitor client progress and adjust vocational plans to support long-term success and recovery

Minimum Requirements

  • Bachelor’s degree in Rehabilitation, Human Services, Psychology, Social Work, or a related field
  • At least 1 year of experience working with individuals with serious mental illness, substance use disorders, or co-occurring conditions
  • Valid driver’s license and reliable transportation
  • Ability to work flexible hours, including community-based services
  • Strong interpersonal, communication, and organizational skills
  • Basic computer proficiency and documentation skills
