Pyramid Healthcare is seeking a dedicated Qualified Professional (QP) to join our Assertive Community Treatment Team (ACTT) in Williamston, NC.
Our ACTT program operates as a "hospital without walls," delivering intensive, community-based behavioral health services to individuals with severe and persistent mental illness. This multidisciplinary team provides comprehensive, person-centered care that addresses mental health, substance use, housing, and vocational needs—meeting clients where they are to reduce crises and improve long-term outcomes.

Develop and implement person-centered treatment plans aligned with best practices
Coordinate care across a multidisciplinary ACTT team
Conduct utilization reviews and ensure compliance with CARF, DHHS, and LME standards
Lead and participate in treatment team meetings
Manage discharge planning and continuity of care
Maintain communication with referral sources and community partners
Complete timely, accurate clinical documentation
Participate in CQI initiatives and peer record reviews
Provide transportation and community-based support for clients
Assist with crisis intervention and emergency service rotation
Bachelor's degree in Social Work, Counseling, Psychology, or related field (required)
Master's degree in a Human Services field (preferred)
Must meet Qualified Professional (QP) criteria per NC regulations (10A NCAC 27G.0104)
1–3 years experience working with severe and persistent mental illness (preferred)
Experience in vocational and/or housing rehabilitation strongly preferred
Valid driver's license required
Strong team collaboration and communication skills
Ability to build meaningful relationships with clients in the community
Sound clinical judgment and professional boundaries
