Acute Intake Coordinator-Valley Vital Care Infusion Pharmacy

Valley Vital Care is an infusion pharmacy located in Waynesboro, VA. We service all of Virginia and West Virginia. We have locations in Winchester, Strasburg, Richmond, Waynesboro, Lynchburg and Roanoke Virginia as well as Charleston, WV. We provide IV therapies to patients in our pharmacy suites as well in the patient's home. Therapies include biologic therapy for chronic conditions, long and short-term antibiotic therapies for self-administration, TPN and enteral feedings.

We are seeking a dedicated and detail-oriented Acute Intake Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing patient intake processes, ensuring that all necessary information is collected accurately and efficiently. This position requires strong communication skills, a solid understanding of health information management, and the ability to work collaboratively within a medical office environment. Experience in benefit investigation, authorizations and health insurances preferred.

Responsibilities

• The Acute Intake Coordinator role is a full-time position responsible for coordinating the discharge plan for new patients entering the pharmacy’s care. This position will incorporate working with healthcare providers, our internal pharmacy team and patients to ensure a seamless transition of care.
• Communicating with hospitals, doctor's offices and referral sources to generate a plan of care for patients.
• Collect and verify patient information, including personal details, medical history, and insurance information.
• Ensure accurate data entry into the electronic health record (EHR) system, maintaining confidentiality and compliance with HIPAA regulations.
• Assist with insurance verification processes to confirm coverage and benefits for patients.
• Conduct comprehensive benefit investigation for new patient referrals and evaluate pharmacy benefit plans and medical benefit plans to help determine out of pocket expenses for patients.
• Communicate effectively with patients regarding their discharge planning and out-of-pocket costs.

Experience

• Previous experience in an intake role is highly preferred.
• An active Pharmacy Technician License is required.
• Familiarity with health information management practices and medical terminology is essential.
• Knowledge of insurance verification processes and hospital operations is highly valued.
• Proficiency in typing and data entry skills to ensure accurate documentation of patient information.
