Job Summary:
The Administrative Assistant – Intake Coordinator plays a key role in the hospice/home health admission process by supporting timely and accurate coordination of patient referrals, verifying insurance benefits, managing documentation, and serving as the first point of contact for patients, families, and referral sources. This position requires excellent organizational, communication, and customer service skills to ensure seamless transitions into hospice care/home health care.
Key Responsibilities:
Patient Intake & Admissions Support
- Receive and process incoming referrals via phone, fax, or electronic systems.
- Enter referral information into EMR or hospice software accurately and promptly.
- Schedule hospice admission visits and coordinate with clinical staff.
- Communicate with referral sources, patients, and families to gather necessary information for admissions.
Insurance & Documentation:
- Verify insurance coverage, including Medicare, Medi-Cal, and private insurance plans.
- Assist in collecting and reviewing necessary documents (e.g., physician orders, consents, face-to-face documentation).
- Ensure all intake paperwork is completed and signed appropriately
Administrative Duties:
- Maintain organized patient files and ensure compliance with HIPAA.
- Answer incoming calls and direct to appropriate personnel.
- Assist with data entry, filing, faxing, and general office duties.
- Support admission staff with follow-ups on pending referrals and physician documentation.
Communication & Coordination:
- Serve as a liaison between families, physicians, nurses, and referral partners during the intake process.
- Notify appropriate team members regarding new admissions and status updates.
- Follow up on missing documents and outstanding orders.
Qualifications:
- High school diploma or equivalent (Associate's or Bachelor’s degree preferred).
- 1-2 years of experience in healthcare administration, preferably in hospice, home health, or medical office setting.
- Strong computer skills.
- Knowledge of hospice regulations and Medicare guidelines is a plus.
- Excellent communication, multitasking, and interpersonal skills.
- Ability to work independently and with a team in a fast-paced environment.
- Bilingual (English/Spanish or other language) preferred but not required.
Work Environment:
- Office-based with some phone communication with patients and families.
- Occasional collaboration with field nurses, physicians, and marketers.
- May require occasional after-hours or weekend support during peak admissions.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
