The Part-Time Admissions Coordinator / Case Manager is responsible for managing the admission process. This position serves as a liaison between hospitals, physicians, families, managed care organizations, and the interdisciplinary team to ensure appropriate placement, regulatory compliance, and continuity of care.
Coordinate the entire admissions process from referral to admission
- Receive and review referrals from hospitals
- Conduct facility tours and provide clear, accurate information regarding services, rates, and clinical capabilities
- Review all clinical documentation to determine eligibility and appropriate placement
- Verify Medicare Part A, Medicare Advantage, Medicaid (NJ FamilyCare), Managed Care, and commercial insurance benefits
- Communicate admission decisions in a timely manner to referral sources
- Maintain census tracking and admission logs
