Admission Coordinator-Case Manager

The Part-Time Admissions Coordinator / Case Manager is responsible for managing the admission process. This position serves as a liaison between hospitals, physicians, families, managed care organizations, and the interdisciplinary team to ensure appropriate placement, regulatory compliance, and continuity of care.


  • Coordinate the entire admissions process from referral to admission
  • Receive and review referrals from hospitals
  • Conduct facility tours and provide clear, accurate information regarding services, rates, and clinical capabilities
  • Review all clinical documentation to determine eligibility and appropriate placement
  • Verify Medicare Part A, Medicare Advantage, Medicaid (NJ FamilyCare), Managed Care, and commercial insurance benefits
  • Communicate admission decisions in a timely manner to referral sources
  • Maintain census tracking and admission logs
