Admissions Coordinator

Whittier Hills Healthcare Center is looking for a motivated Admissions Coordinator to join our team!

Responsibilities:

• Working alongside our Community Liaison .
• Coordinating the transfer of patients in the Hospital and Acute setting to the SNF setting
• Communicating efficiently with team regarding new admissions.
• Providing tours to interested families in need of skilled nursing facility placement for their loved one.
• Meeting with new residents and their families for the completion of the Admissions Agreement packet.
• Maintaining relationships with our community partners such as: Physicians, Social Workers, Case Managers, and Discharge Planners.
• Verifying insurance benefits and eligibility

Education/Requirements:

• 1 year experience as an Admission Coordinator (Preferred)
• Knowledge on SNF setting (Preferred)

Pay: $28.00 - $38.00 per hour
