Admissions Liaison Nurse RN - Weekends

WEEKEND RN ADMISSIONS/HOSPITAL LIAISON NURSE NEEDED

WORK SCHEDULE: 24 hours a week in three 8-hour shifts Saturday, Sunday & Monday. This position requires travel in your personal vehicle with mileage reimbursement. Will also be part of the on-call rotation twice per month.

ABOUT THE POSITION: Under the direction of client’s primary care physician and Hospice Medical Director, the Admissions Nurse (RN) is a member of the Interdisciplinary Team. The Admissions Nurse is responsible for the “Initial Nursing Assessment” and initial Plan of Care development. Under the direction of the client’s primary care physician and Hospice medical director, the RN plans and provides the initial individualized skilled nursing care to enable patients to participate as fully as possible in the activities of daily living, in both in-home and in-patient settings. The RN participates as a member of the Interdisciplinary team (IDT) to plan and provide holistic care to patients and families. Nursing care reflects observations, interventions, and goals that are in accordance with the patient and family and the IDT. This nurse will provide communication to the assigned Primary Team to facilitate continuity of care.

Additionally, as the Hospital Liaison, one of the primary responsibilities of this position is building and sustaining community relationships in the acute care setting, which include Kootenai Health and NIACH, as well as Shoshone and Benewah acute care facilities as appropriate. This position will also act as the primary nurse until the patient is admitted to Hospice Services and provide end of life education and resources to the staff in the acute care setting. The RN will provide continuity from the acute care setting to the home, facility or the Schneidmiller House. The nurse will communicate client assessment and recommended care measures to the Admit Team or primary nurse receiving the patient. When acting in the emergent care capacity, the RN will provide prompt response to physical and emotional needs of patients and their families in the form of phone call and/or home visits during regular day shift hours; they screen a variety of questions, problems, and crisis situations that may arise.

MINIMUM QUALIFICATIONS: RN required; current Idaho State RN in good standing; minimum six (6) months nursing experience; current CPR; valid driver’s license, personal vehicle for on-the-job transportation, and evidence of insurance coverage as required by HONI policy required.
