Job Overview
We are seeking a dedicated and compassionate Admissions Nurse to join our healthcare team. The Admissions RN is responsible for managing all aspects of the patient intake process including establishing and maintaining positive relationships with hospice staff, patients, nursing facilities, vendors, and referral sources. This RN position requires a strong foundation in medical documentation, patient assessment, and experience in diverse specialties such as critical care, geriatrics, home health, and end of life care. The Admissions Nurse will collaborate closely with multidisciplinary teams to facilitate efficient patient transitions and uphold the highest standards of patient care and confidentiality. This position will travel to Ulster or Dutchess, depending on where the admission is.
What you will do:
- Perform the initial patient/family physical, emotional, psychosocial, cultural, and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic hospice services.
- Document all aspects of pre-admission activities, evaluation, and initial assessment thoroughly in the EMR system, ensuring all clinical, medical, emotional, psychosocial, and physical attributes are captured accurately.
- Follow CMS and agency guidelines to assist the hospice team and Medical Director in determining and supporting clinical eligibility for hospice services.
- Perform thorough comprehensive assessments and communicate findings to hospice team members through interpersonal communication and documentation within the EMR.
- Initiate discussions regarding advanced directives and code status, providing information and answering patient/family questions.
- Provide anticipatory guidance on disease trajectory and end-of-life symptom management to patients and families to empower them to cope with events as they occur throughout the end-of-life process.
- Acts as a resource to introduce and arrange for the provision of complimentary therapies as appropriate.

Work Environment:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job
- Working conditions include working within assigned settings, including but not limited to Hudson Valley Hospice offices, hospitals, patient/family homes, nursing homes, and assisted living facilities
- Ability to carry up to twenty-five pounds of supplies and/or materials.
- Routine duties involve potential for direct exposure to blood or body fluids.
- Capability to work a flexible schedule to meet the needs of the patients including occasional weekends and holidays.
- Ability to provide transportation for self and access public buildings to provide community education.
- Ability to drive and work in winter weather including snow
- Valid driver’s license in New York State.
Preferred Education & Experience:
- Graduate from accredited nursing program
- Valid Nursing License in New York State.
- Experience working in a fast-paced environment preferred
- A minimum of two years experience in home care or hospice preferred.
- Experience and comfort in caring for terminally ill patients.
Hours: Full Time - 40 hours per week; (4) 10hr shifts from 8:30am-6:30pm (Rotating Schedule); must have availability to work weekends and holidays when needed
Pay: $80,000-90,000/year
FLSA: Exempt
