Modern Recovery Services offers a variety of services to assist individuals and families through the process of mental health, addiction, treatment, and recovery from a variety of the challenges clients face in life. This Case Manager position is for Modern Recovery Services PHP facility for adolescents seeking addiction and mental health services.

The Case Manager role is to support those in recovery from a mental health disorder and/or co-occurring disorders. The Case Manager will serve as a role model, mentor, advocate and motivator to recovering individuals in order to help prevent relapse and promote long-term recovery. The Case Manager will work to develop authentic peer to-peer relationships while maintaining professional boundaries. The Case Manager is responsible for the direct supervision and for the safety of all clients. They participate in program schedule facilitation adherence. They support the environment of care to be conducive to client safety and treatment.

Job Duties:
  • Facilitate Groups
  • Provide recovery education to clients for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance.
  • Assist recovering persons to identify their personal interests, goals, strengths and weaknesses regarding recovery.
  • Assist/coach recovering persons develop their own plan for advancing their recovery.
  • Facilitate the transition from professional services to community based services.
  • Promote self-advocacy by assisting recovering persons to have their voices fully heard; their needs, goals and objectives established as the focal point of rehabilitation and clinical services.
  • Provide hope and encouragement for opportunities at varying levels of involvement in community based activities (e.g., work, school, relationships, physical activity, self-directed hobbies, etc.)
  • Participate in documentation in electronic health record
  • Maintain client confidentiality HIPAA compliance
  • Manage crisis situations using Voluntary Time-Out and verbal de-escalation interventions
  • Communication with team members the status of clients in verbal and written form

Qualifications:

  • High school diploma/G.E.D OR 2 years' experience in the behavioral health field.
  • DPS fingerprint clearance card (preferred- will be required to obtain)
  • CPR/First Aid certification (preferred- will be required to obtain)
  • Current TB test (will be required to obtain)

Recommended Qualifications:

  • Experience facilitating groups.
  • Preferred behavioral health experience.
  • Individual must have an understanding of and respect for each individual's unique path to recovery.
  • Must be able to speak English, hear, use a telephone, and operate standard office equipment, such as a computer, copier, and fax machine.

Job Type: Full-time

Pay: $45,000.00 per year
