Position Summary

The CIS Case Manager/MHRT-C provides community-based support services under MaineCare Section 17. The role focuses on helping members build independent living skills, access resources, and progress toward individualized recovery goals.

Essential Duties and ResponsibilitiesAssessment & Planning

  • Conduct intake and ongoing assessments, including LOCUS as assigned
  • Gather psychosocial history and assess member strengths and needs
  • Develop and maintain person-centered service plans
  • Ensure plans reflect medical necessity and member goals
  • Update plans within required timelines

Care Coordination

  • Coordinate services with medical, behavioral health, and community providers
  • Facilitate referrals and linkage to needed resources
  • Support transitions of care and discharge planning
  • Maintain regular communication with members and collateral contacts

Service Delivery & Monitoring

  • Provide community-based case management and skills support
  • Monitor member progress toward goals
  • Identify barriers and implement appropriate interventions
  • Support community integration and independent living skills
  • Respond appropriately to member crises and safety concerns

Documentation & Compliance

  • Complete documentation accurately and within required timeframes
  • Maintain LOCUS documentation when applicable
  • Ensure records meet MaineCare and OBH standards
  • Maintain HIPAA compliance and confidentiality
  • Participate in audits and quality assurance activities

Team Participation

  • Participate in required clinical supervision
  • Attend team meetings and case reviews
  • Collaborate with the clinical supervisor regarding medical necessity
  • Complete required trainings and professional development

Qualifications

Required

  • High school diploma or equivalent required
  • Active MHRT-C certification
  • Valid driver’s license, reliable transportation, and ability to travel within the community
  • Ability to pass required background checks in accordance with MaineCare and agency policy
  • Basic computer skills and ability to complete electronic documentation
  • Strong organizational, communication, and time-management skills
  • Ability to maintain professional boundaries and confidentiality (HIPAA compliance)

Preferred

  • Bachelor’s degree in social work, psychology, behavioral health, or related field
  • Experience providing community-based behavioral health or case management services
  • Knowledge of MaineCare Section 17 Community Integration Services
  • Experience working with diverse, immigrant, or multilingual communities
  • Bilingual or multilingual skills (French, Somali, Arabic, or other languages)

Pay: $30.00 - $35.00 per hour

Expected hours: No less than 20.0 per week
