Position Summary
The CIS Case Manager/MHRT-C provides community-based support services under MaineCare Section 17. The role focuses on helping members build independent living skills, access resources, and progress toward individualized recovery goals.
Essential Duties and ResponsibilitiesAssessment & Planning
- Conduct intake and ongoing assessments, including LOCUS as assigned
- Gather psychosocial history and assess member strengths and needs
- Develop and maintain person-centered service plans
- Ensure plans reflect medical necessity and member goals
- Update plans within required timelines
Care Coordination
- Coordinate services with medical, behavioral health, and community providers
- Facilitate referrals and linkage to needed resources
- Support transitions of care and discharge planning
- Maintain regular communication with members and collateral contacts
Service Delivery & Monitoring
- Provide community-based case management and skills support
- Monitor member progress toward goals
- Identify barriers and implement appropriate interventions
- Support community integration and independent living skills
- Respond appropriately to member crises and safety concerns
Documentation & Compliance
- Complete documentation accurately and within required timeframes
- Maintain LOCUS documentation when applicable
- Ensure records meet MaineCare and OBH standards
- Maintain HIPAA compliance and confidentiality
- Participate in audits and quality assurance activities
Team Participation
- Participate in required clinical supervision
- Attend team meetings and case reviews
- Collaborate with the clinical supervisor regarding medical necessity
- Complete required trainings and professional development
Qualifications
Required
- High school diploma or equivalent required
- Active MHRT-C certification
- Valid driver’s license, reliable transportation, and ability to travel within the community
- Ability to pass required background checks in accordance with MaineCare and agency policy
- Basic computer skills and ability to complete electronic documentation
- Strong organizational, communication, and time-management skills
- Ability to maintain professional boundaries and confidentiality (HIPAA compliance)
Preferred
- Bachelor’s degree in social work, psychology, behavioral health, or related field
- Experience providing community-based behavioral health or case management services
- Knowledge of MaineCare Section 17 Community Integration Services
- Experience working with diverse, immigrant, or multilingual communities
- Bilingual or multilingual skills (French, Somali, Arabic, or other languages)

Pay: $30.00 - $35.00 per hour
Expected hours: No less than 20.0 per week
