- Provide regular and consistent services and supports to clients identified appropriate for community supports
- Management of all aspects of clinical treatment including referrals to additional resources, crisis assessment and support as needed
- Work with client on skill building, activities that support stabilization in living environment, connection to community resources and natural supports
- Service provision shall be conducted primarily in the client’s living and other environments as necessary or appropriate
- Provide outreach and engagement when needed
- Closely collaborate with other care teams within Polara to coordinate and meet client needs (Inpatient Hospital, Crisis Stabilization Unit, Outpatient Services, Primary Care)
- Facilitate Adult Recovery Team Meetings (ARTs)
- Participates as a team member and attends scheduled staffings and supervision.
- Partners with client and team to identify treatment goals
- Maintains clinical record per agency policy and procedure.
- Attends all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainings
- Management of all aspects of treatment, including referrals to additional resources when appropriate.
- Performs other duties as assigned
- 21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member’s scope of practice; or minimum 18 years old for select administrative positions
- Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
- Must possess and maintain a valid Driver License, current automobile insurance, and a driving record that meets the guidelines and requirements of the organization
