Adult Foster Care Case Manager

Adult Foster Care seeking Case manager to do home visits and assessments within the community.

FEE FOR SERVICE. Positions FLEXABILITY allows for you to schedule your clients when works best for you. No over time. Paid gas millage.

Gabriel Care Adult Foster Care has been in business since early 2000s and we service the entire state of Massachusetts.

A driver's license, a car and a cell phone are necessary for the position. DRIVING IS REQUIRED

You will be given an iPad and/or laptop to use to do your notes, scheduling, case management and assessments on

You will be given a caseload with clients that need home visits either monthly or bimonthly. The number of hours you wish to work determines the number of clients on your caseload.

FLEXABILTY. Scheduling is done by you.

Summary

As an Adult Foster Care Case Manager, you will be responsible for coordinating comprehensive care for patients in a supportive environment. Reporting to the program director, your role will involve utilizing core skills in patient assessment, managed care, and discharge planning to ensure optimal patient service and care. Your premium experience in critical care and geriatrics will enhance the quality of care provided. Additionally, your relevant skills in both inpatient and outpatient settings will be essential in fostering a holistic approach to patient well-being.
