Adult Mental Health Case Manager

Independent Management Services of Minnesota, Inc. is looking for a dedicated and compassionate Adult Mental Health Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive case management services to individuals with mental health issues. This role involves assessing clients' needs, developing individualized care plans, coordinating services, and monitoring progress. The Adult Mental Health Case Manager will work closely with clients, their families, and other healthcare professionals to ensure that clients receive the necessary support and resources to improve their mental health and overall well-being. The successful candidate will have a strong understanding of mental health disorders, excellent communication skills, and the ability to work effectively in a team-oriented environment. This position requires a high level of empathy, patience, and the ability to handle sensitive information with confidentiality. The Adult Mental Health Case Manager will also be responsible for maintaining accurate and up-to-date records, advocating for clients' needs, and providing education and support to clients and their families.

Responsibilities

  • Assess clients' mental health needs and develop individualized care plans.
  • Coordinate and monitor the delivery of mental health services.
  • Provide crisis intervention and support to clients in need.
  • Advocate for clients' needs and ensure they receive appropriate services.
  • Maintain accurate and up-to-date client records.
  • Collaborate with healthcare professionals, social workers, and other stakeholders.
  • Conduct regular follow-up meetings with clients to monitor progress.
  • Provide education and support to clients and their families.
  • Assist clients in accessing community resources and services.
  • Develop and maintain relationships with community organizations and service providers.
  • Participate in case conferences and team meetings.
  • Ensure compliance with all relevant laws and regulations.
  • Provide support and guidance to clients in developing coping strategies.
  • Monitor clients' medication adherence and provide education on medication management.
  • Conduct home visits and outreach as needed.
  • Develop and implement discharge plans for clients transitioning out of services.
  • Participate in ongoing training and professional development.
  • Maintain confidentiality and handle sensitive information with care.
  • Prepare reports and documentation as required.
  • Evaluate the effectiveness of care plans and make necessary adjustments.

Requirements

  • Bachelor's degree in social work, psychology, or a related field.
  • Licensed Social Worker (LSW) or equivalent certification.
  • Minimum of 2 years of experience in mental health case management.
  • Strong understanding of mental health disorders and treatment options.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-oriented environment.
  • High level of empathy, patience, and compassion.
  • Strong organizational and time management skills.
  • Ability to handle sensitive information with confidentiality.
  • Proficiency in Microsoft Office and case management software.
  • Valid driver's license, car insurance, and reliable transportation.
  • Ability to conduct home visits and outreach as needed.
  • Strong problem-solving and critical thinking skills.
  • Ability to work with diverse populations.
  • Knowledge of community resources and services.
  • Ability to advocate for clients' needs effectively.
  • Strong documentation and record-keeping skills.
  • Ability to manage a caseload and prioritize tasks.
  • Must pass background check.
