Overview
We are seeking a dedicated and compassionate Autonomous Nurse Practitioner to join our mobile healthcare team 3-5 days a week. The ideal candidate will provide high-quality patient care across various settings. This role requires a strong commitment to patient wellness, effective communication skills, and the ability to work collaboratively within a multidisciplinary team.

Duties

• Conduct comprehensive assessments of patients, including physical exams and health histories.
• Develop and implement individualized care plans based on patient needs.
• Manage patients with chronic conditions.
• Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times.
• Educate patients and families on health management, disease prevention, and wellness strategies.
• Participate in utilization management to optimize patient outcomes and resource allocation.
• Adhere to infection control protocols to ensure a safe environment for patients and staff.
• Collaborate with other healthcare professionals to coordinate comprehensive care.

Experience
Candidates should possess the following skills:

• Strong background in patient care, particularly in wound care and critical care settings.
• Proficiency in medication administration and knowledge of infection control practices.
• Familiarity with behavioral health issues and treatment modalities.
