MA Care Partners is seeking a Nurse Practitioner (NP) to join our Medicare Advantage-focused primary care team in Nevada!

Pay: $120,000.00 - $150,000.00 per year plus a Bonus structure

The primary clinic location for this position is Fernley, NV but the position is hybrid; a mixture of clinic days, home visit days, telemedicine is flexible and can be determined collaboratively based on panel needs and provider preference.

MA Care Partners specializes in caring for Medicare Advantage members, with a focus on complex, high-acuity geriatric patients. We deliver care across multiple settings — clinic-based visits, in-home visits, and telemedicine — with a strong emphasis on comprehensive health assessments. In this role, you will conduct primary health assessments (similar in scope to Medicare Annual Wellness Visits) across all three visit modalities. Over time, you will have the opportunity to build and manage your own primary care panel of Medicare Advantage patients.

Qualifications to fulfill role as a Nurse Practitioner must include:

• Must be a licensed Nurse Practitioner (NP) in the state of Nevada – or be eligible for a Nevada nursing license. Please note: Physician Assistants (PAs) are not eligible for this role at this time.
• Certified by a national Nurse Practitioner certifying body (ANCC or AANP)
• Strong working knowledge of HCC (Hierarchical Condition Category) coding and risk adjustment methodology, including CMS-HCC and RxHCC models
• Familiarity with HEDIS measures, Medicare Advantage quality programs, and Stars ratings
• Experience or strong interest in geriatric medicine and/or Medicare Advantage primary care
• Comfortable working independently across multiple care settings including clinic, home visits, and telehealth
• Experience conducting comprehensive health assessments, wellness evaluations, or Annual Wellness Visits preferred
• Commitment to thorough, compliant documentation with emphasis on MEAT criteria and accurate ICD-10 coding to support risk adjustment
