Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews in both English and the target language to ensure clear understanding.
- Develop, implement, and evaluate individualized care plans aligned with best practices in acute care, geriatrics, pediatrics, or specialty areas such as gastroenterology or behavioral health.
- Administer medications and treatments such as IV infusions, botulinum toxin injections, dermal fillers, or esthetic laser procedures following strict safety protocols.
- Collaborate with physicians, specialists, and multidisciplinary teams utilizing EMR systems like eClinicalWorks to document patient encounters accurately and efficiently.
- Provide patient education on health management strategies, including chronic disease management, nutrition counseling, or post-acute care instructions.
- Assist in procedures requiring sterile technique such as suturing, catheterization, spinal taps, or specimen collection while maintaining infection control standards.
- Participate in clinical research projects or quality improvement initiatives to enhance patient outcomes and uphold compliance with HIPAA and NCQA standards.
- Bilingual proficiency in English and Spanish with excellent verbal and written communication skills.
- Extensive experience with EMR/EHR systems such as eClinicalWorks for documentation and order entry.
- Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques.
- Proficiency in performing advanced procedures including IV insertion, venipuncture, tube feeding management, and sterile processing.
- Familiarity with family medicine.
- Ability to manage acute pain cases.
- Experience with CPT coding and ICD coding (ICD-9/10) for accurate documentation and billing processes.
- Demonstrated capability in patient assessment for complex cases involving dialysis patients, ICU or trauma center experience preferred.
- Skills in health coaching, discharge planning, case management, or working with individuals with disabilities or developmental challenges are advantageous.
Pay: $105,000.00 - $120,000.00 per year
