Job Summary: Provides high-quality culturally competent whole person primary care to patients of Mazzoni Health Center in alignment with Mazzoni Center's mission & vision.
Communication: Good oral and written communication skills are required for this position. The person in this position must be able to communicate effectively with the patients, colleagues, staff and all members of the Mazzoni organization demonstrating respect and contributing to a positive environment.
Key Responsibilities:
Delivery of Medical Care

Provides direct patient care to ambulatory patients in primary care setting – including but not limited to the following: obtain comprehensive relevant medical histories; diagnose, assess and treat acute and chronic medical problems; recommend preventive care; perform office-based procedures, patient education and counseling; facilitate and offer continuity of care; and refer to specialty and/or higher acuity care as clinically indicated.

Notifies supervising physician/ collaborating physician regarding care of patients requiring direct physician consultation

Review external medical records and consultation reports to ensure continuity of care.

Partner with front desk, nursing, and medical support staff to triage patient calls and advise on patient concerns as necessary.

Participate in after hour on-call rotation.

Respond to on-site medical emergencies as an actively certified Basic Life Support (BLS) Health Care Professional when indicated.

Ensure complete and timely documentation of care within the electronic health record (EHR) in accordance with Mazzoni Center policies (within 72 hours of provision of care).

Coordinate care with other team members (internal and external, including registered nurses, behavioral health consultants, medical case managers, mental health therapists, etc.) to facilitate comprehensive care.

Remain aware and in compliance with all institutional, insurance, HRSA, grant-based and internal quality measures.

Participate in organizational and departmental meetings including case conferences and educational lectures.

Exhibit dedication to lifelong learning via maintenance of CME credits and/or educational requirements as per Mazzoni Center, State, and Specialty Board requirements.

Precept/mentor learners and engage in educational activities including review of advance practice provider note sign off and, when indicated, direct clinical supervision.

Communicate with Medical Director in timely manner regarding schedule changes, time off, reimbursement requests, licensure and other matters relating to practice operations.

Compete regular peer chat reviews and provide structured feedback.

Exhibit civil, professional and inclusive behaviors towards all members of staff, patients, and Mazzoni Center community recognizing one's role as a Mazzoni Center clinical leader.

Education, Training, License and Certification, Skills and Experience:
-
Graduate of an accredited nurse practitioner or physician assistant graduate program.

Hold active professional licensure as registered nurse and certified registered nurse practitioner; OR hold active professional licensure as a certified physician assistant in the state of Pennsylvania.

Maintain an active DEA license
-
Maintain active and accurate supervisor/collaborative physician agreements in the online Pennsylvania Licensing System (PALS) with actively employed Mazzoni physicians as assigned by the Mazzoni Medical Director or Chief Medical Officer as required by the state of Pennsylvania based on type of professional licensure.
Experience & Skills:

Primary care experience required

Experience in gender affirming care, sexual health care, HIV care preferred

Must possess technological literacy with general principles of EHR and basic computer skills

Must be committed to practice in accordance with Mazzoni Center's mission and demonstrate commitment to the LGBTQ+ community

Clearances:

Background checks are required, including Criminal Background Clearance, Child Abuse Clearance, and FBI Clearance in Pennsylvania and state of residence.
Physical Requirements:
1. The ability to stand, sit and walk for extended periods of time
2. The ability to lift objects greater than 35 pounds
3. The ability to communicate using written and spoken words
4. Must demonstrate visual and auditory acuity
5. Must have all necessary vaccinations
