Advanced Practice Provider

We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team. This role provides advanced care and physician support during the treatment of patients in our clinic, dialysis and hospital settings. This position will average 32 hours per week and will focus primarily on the clinic setting, but some hospital and dialysis rounds may be required.

Key Responsibilities:

• Collaborate with nephrologists and hospital teams to coordinate patient care
• Perform follow-ups and manage chronic kidney disease (CKD) patients
• Provide outpatient nephrology care in our clinic as needed
• Educate patients on kidney health, treatment plans, and disease management
• May conduct hospital and dialysis rounds on nephrology patients

Requirements

• Current certification as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant).
• Strong knowledge of HIPAA regulations and patient privacy standards.
• Nephrology experience preferred
• Excellent communication skills and the ability to work collaboratively in a team environment.

Pay: $90,000.00 - $112,000.00 per year
