We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This advanced practice provider will be focused on the well-being of Onsite employees and their beneficiaries, at AMPG (6925 South Harding, Indianapolis, IN 46217).

Highlights

Community Health Network has an excellent opportunity for an experienced, family medicine certified advanced practice provider to practice in an Employer Health Onsite primary care role. Minimum two years of experience as an advanced practice provider is required.

Job Duties

• 
  Routine check-ups and preventive care

• Diagnosis and treatment of illness or injury

• Managing chronic or complex health conditions

• Medication management

• Referral to specialists for more serious conditions

Education

• Educational background consistent with requirements for licensure and certification with the State of Indiana
