Compensation: $100,000 - $130,000 base salary range, plus eligibility for bonus, comprehensive benefits including medical, dental, vision, 401(k), PTO, and $3,000 CME

Our breakthrough care model for Medicare patients combines relationship-driven primary care and specialty care into simultaneous visits. Each patient is supported by a dedicated Medical Assistant – a concierge partner who builds long-term trust, eases caregiver burden, and leverages AI-enabled workflows to improve chronic disease outcomes. 

The Opportunity

Town Square Health is hiring a Primary Care Provider (NP) in the Rochester area to deliver high-quality primary care in a Medicare-focused, value-based care environment. This is a clinic-based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20–40-minute visit lengths based on patient needs, and a team-based care model with two Medical Assistants paired with each provider.

You will play a key role in delivering excellent clinical care, coordinating across the care team, and helping shape a new care model designed to improve outcomes and patient experience.

Qualifications

• Nurse Practitioner (NP), DNP or equivalent experience strongly preferred.
• National certification in at least one of the following specialties:
  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner
• 2-3 years minimum experience as an NP 
• Active New York State Nurse Practitioner licensure in good standing or must obtain prior to start date
• Applied for DEA registration prior to hire/start date
• Must meet all applicable employer and payer credentialing requirements
• Ability to meet post-offer requirements, including drug screen and health assessment
• Compliance with all New York State health requirements
