Advanced Practice Registered Nurse (APRN)

Caritas In-Home Primary Care is a family focused medical practice seeking to maximize the quality of life for homebound/home-limited patients by providing primary care in their home. This is a full-time position to cover patients located in the southern part of Clay county and western part of Putnam county with flexible scheduling, no weekends, no holidays and limited on-call responsibilities. Areas primarily covered: Middleburg, Keystone Heights, Interlachen and Florahome. Benefits include PTO, 401(k) match after one year of employment, mileage reimbursement and a medical stipend. A successful candidate will provide each patient with compassionate and responsive care and will communicate with the healthcare team internally and externally to help patients meet optimal health status and prevent hospitalizations.

Pay: $85,000.00 - $140,000.00 per year
