Job Summary
Boxton Cares, Inc. is seeking a compassionate and mission-driven Nurse Practitioner (NP) to join our community-based healthcare clinic. The Nurse Practitioner will provide comprehensive clinical care with a focus on HIV prevention, infectious disease management, sexually transmitted infection (STI/STD) screening, and community health services.
This role is vital in advancing our mission to improve health equity, expand access to testing and treatment, and support underserved populations through high-quality, patient-centered care. (Boxton Cares, Inc.)
The NP will work collaboratively with clinical staff and community partners to deliver preventive, diagnostic, and treatment services.
Duties
- Clinical Care
- Perform comprehensive patient assessments including medical history, physical examinations, and risk evaluations.
- Diagnose and treat acute and chronic conditions related to infectious diseases and sexual health.
- Provide HIV testing, STI screening, counseling, and treatment services.
- Prescribe medications including PrEP, PEP, and other appropriate therapies in accordance with state regulations.
- Develop and implement patient care plans for ongoing treatment and disease management.
Preventive & Community Health
- Provide education and counseling on HIV prevention, sexual health, and risk reduction strategies.
- Support community health initiatives focused on disease prevention and early detection.
- Participate in outreach programs aimed at increasing awareness and testing within the community.
Care Coordination
- Collaborate with case managers and healthcare professionals to ensure continuity of care.
- Coordinate referrals to specialty care, behavioral health, and social services when needed.
- Support patients with medication adherence and ongoing treatment plans.
Documentation & Compliance
- Maintain accurate and timely medical documentation within the electronic health record (EHR).
- Ensure compliance with HIPAA, CDC guidelines, and Florida healthcare regulations.
- Participate in quality improvement and clinical protocol development.
Patient Engagement
- Provide culturally competent, non-judgmental care to individuals from diverse backgrounds.
- Build trust with patients by fostering a supportive and inclusive healthcare environment.
Experience
- Education
- Master’s or Doctorate degree from an accredited Nurse Practitioner program.
Licensure
- Current Florida Advanced Practice Registered Nurse (APRN) license.
- National certification (FNP, ANP, or AGNP).
- Active DEA license (or eligibility to obtain).
Preferred experience includes:

Pay: $110,374.79 - $144,967.48 per year
