Patient Load: Will vary by facility and payer mix, but the target remains 25–35 patients per day

Overview: We are seeking a dedicated Advanced Registered Nurse Practitioner (ARNP) to join our healthcare team. The ARNP will deliver both inpatient and outpatient care with a focus on cardiovascular health, working in nursing homes, long-term care, rehabilitation centers, and skilled nursing facilities. This role involves comprehensive patient care, diagnosis, treatment planning, health maintenance, and preventive care. The candidate must be well-versed in all relevant healthcare policies and regulations.

Responsibilities:

  • Conduct patient assessments, including obtaining medical history and performing physical exams.
  • Diagnose and develop treatment plans for acute, chronic, and cardiovascular conditions.
  • Provide direct patient care, counseling, and follow-up for treatment adherence.
  • Prescribe medications and therapies following state guidelines and professional practices.
  • Collaborate with attending physicians, specialists, and facility staff to ensure continuity of care.
  • Perform routine check-ups and monitor patient progress, documenting care in electronic medical records (EMR).
  • Manage patient discharges and perform post-acute care evaluations.
  • Educate patients and families about preventive health, including lifestyle and disease management.
  • Ensure compliance with infection control protocols and other healthcare regulations.
  • Participate in continuing education to remain up-to-date with medical advancements.

Qualifications:

  • Education: Bachelor's, Master’s, or Doctoral degree in Nursing.
  • Certifications: Advanced Registered Nurse Practitioner (ARNP) with a Florida license and Board; BLS/ACLS Certification, DEA license number required
  • Experience: 3+ years of experience in a cardiovascular setting and 3 years of experience in nursing home setting. (Preferred)
  • Proficiency in using EMR/EHR systems, Google Workspace, and Microsoft Office.
  • Bilingual (English/Spanish) preferred.
