Agency Registered Nurse

Community Home Care LLC [Est. 2021] is a Medicaid-Medicare agency provider specializing in Personal Care, Attendant Care, and Homemaker Services. Staff responsibilities comprise assisting members with Activities of Daily Living [ADL] such as:

  • Eating and Meal Preparation,
  • Toileting, Bathing, Personal Hygiene, and Dressing
  • Mobility and Bed Transfers
  • Laundry and Housework
  • Shopping

Agency Registered Nurse responsibilities comprise:

  • Overseeing and documenting Agency service delivery as a component of the Quality Assurance Program.
  • Supervising staff provision of services, if necessary.
  • Ensuring Agency compliance pertaining to documentation, deadlines, and service delivery procedure.
  • Ensuring Agency participants’ service plans are developed within 30 days from the date of Medicaid assessment.
  • Reviewing and at least annually updating participants’ service plans to ensure that needs are accurately and understandably reflected.
  • Reviewing staff progress notes and electronic verification data periodically to ensure services are delivered in accordance with the service plan and level of care assessment.
  • Notifying the Department of Health and Welfare of any significant changes in participants’ conditions as noted by staff during service delivery, using the Medicaid significant change form.
  • Conducting interviews in the event that a significant change in the participant’s condition is reported.
  • Conducting interviews with the participant as necessary, but at least annually, to evaluate wellbeing.
  • Notifying the Department of Health and Welfare if service delivery is deficient or if the participant exhibits indications of abuse, neglect or exploitation.
  • Being reasonably accessible during business hours to the Agency, participants, caregivers and the Department of Health and Welfare to respond when care issues arise.
  • Participating in and documenting the training of new caregivers.

Time will be primarily spent conducting phone interviews, creating service plans, training staff, and auditing documentation. The expected hours per week are ~10-25 and may negotiated as per requested weekly workload.

Contract compensation is $30 hourly; $35 hourly may be invoiced in the event of less than 10 hours worked weekly. A $350 initial signing and re-signing bonus is stipulated within the contract.
