Aging Case Manager

JOB DESCRIPTION

TITLE: Case Manager

REPORTS TO: Director of Senior Services

GENERAL RESPONSIBILITIES: The incumbent will act as a representative of all the programs dedicated to providing services to the elderly in Levy County.

SPECIFIC RESPONSIBILITIES:

1. Receive referrals for all applicable programs.

2. Make home visits and assess clients.

3. Prepare and implement case plans, supervise and review as required by program.

4. Responsible for complete Case Records/provide follow-up as needed.

5. Work closely with other service professionals in providing services to clients to prevent overlapping or duplication of services.

6. Be willing to take required training.

7. Schedule service delivery staff to ensure no interruption of services to the clients.

8. Supervise direct service staff.

9. Perform other duties as assigned by the supervisor.

AUTHORITY AND ACCOUNTABILITY: The incumbent is delegated full authority necessary for the effective performance of the responsibilities assigned, within the restrictions of applicable SREC policies and instructions, and is fully accountable to the supervisor for his/her performance.

DESIRABLE QUALIFICATIONS:

1. A four year college degree from an accredited institution with course work in social work, sociology, psychology, nursing, food service and/or related fields.

2. Two years experience in a nursing related field.

3. Experience as required above may be substituted on a year-to-year basis for the college requirement.

SPECIAL QUALIFICATIONS:

Bondable

Must pass Background and Drug Screening Test.

Must have valid driver's license.

Must have valid vehicle insurance.

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
