TARCOG is seeking dynamic Alabama Cares Case Managers to join our team. We are looking for individuals who share our core values of service, initiative, accountability, and transparent communication.
SUMMARY DESCRIPTION: Responsible for implementation of the Alabama Cares Program within the TARCOG area following specific requirements as set out in contract agreements entered into with other parties in the course of the operation of the program.
SUPERVISORY CONTROLS: Receives general instructions from the Alabama Cares Program Coordinator. Supervisor sets limitations, quality and quantity expected, deadlines and priorities; employee uses initiative in carrying out assignments; and work is checked for technical accuracy.
RESPONSIBILITIES AND DUTIES:
- Review and screen referrals to determine if clients meet Alabama Cares Program eligibility requirements.
- Communicate with ADRC staff.
- Interview and enroll eligible clients for services through Alabama Cares Program utilizing approved program tools.
- Assess client needs, develop care plans, authorize services, arrange services, coordinate the provision of services among providers, and follow-up as needed.
- Monitor client care plan goals.
- Assist in determining progress and needs of clients and furnish input for case management.
- Act as an Information and Referral source for clients needing access to community services.
- Conduct client reassessments as per program requirements.
- Establish service delivery for Alabama Cares Program, coordinating services being provided and arranging resources.
- Observe and evaluate treatments and responses; advocate for needed services and entitlements; obtain additional resources; intervene in crises; provide personal support.
- Manage waitlists for Caregiver Support Program.
- Maintain clients' records by reviewing case notes, logging events and progress.
- Monitor clients' progress by conducting routine follow-ups and sharing results and obstacles to supervisor as needed.
- Assist with outreach projects, education, and special events to target program audiences.
- Participate in required meetings/trainings.
- Follow TARCOG and program policy and procedures.
- Maintain accurate electronic client records utilizing approved program software.
- Complete program work using Microsoft Office Suite (i.e., Excel, Word, Teams, Calendar).

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
- Knowledge of social work programs and processes.
- Knowledge of community resources and support network available to clientele served.
- Ability to communicate orally and in writing to exchange information with co-workers, clients, service agencies, state agency personnel, etc.
- Ability to relate to the elderly and their unique problems.
- Knowledge and ability to write and print legibly.
- Knowledge and ability to operate office equipment including telephone, copy machine, fax machine, computer, and calculator.
- Knowledge and ability to do basic arithmetic.
- Ability to read and interpret guidelines and regulations and apply them to the job.
- Completion of Initial Case Manager Training within three (3) months of employment.
- Bachelor’s Degree from an accredited four-year college or university, preferably in the field of social services, psychology, or a related field.
- At least three (3) months of work experience in a social work or related program.
- A valid driver’s license and a good driving record.
