Registered Nurse (RN) – SFC Nurse
Position Summary
The Registered Nurse (RN) serves as the clinical supervisor for Structured Family
Caregiving (SFC) participants under the Elderly and Disabled Waiver Program. The RN is
responsible for comprehensive assessments, care plan development, caregiver
oversight, and clinical compliance in accordance with Medicaid waiver requirements
and Atlas ProCare policies.
Key Responsibilities
Clinical Oversight & Assessment
- Conduct initial and ongoing comprehensive nursing assessments of SFC
members in the home environment
- Develop, implement, and update individualized care plans in collaboration with
case managers and physicians
- Evaluate member eligibility and appropriateness for SFC services
- Monitor health status, functional changes, and risk factors
Care Plan Management
- Create person-centered care plans aligned with Structured Family Caregiving
service requirements
- Ensure care plans reflect ADL/IADL needs, supervision levels, and caregiver
responsibilities
- Update care plans at required intervals or upon significant change
Caregiver Supervision & Training
- Train family caregivers on:
o Infection control
o Medication reminders (not administration unless authorized)
o ADL assistance techniques
- Provide ongoing supervision and performance evaluation of caregivers
- Ensure caregivers are adhering to approved care plans and documentation
standards
Compliance & Documentation
- Maintain accurate, timely documentation in accordance with Georgia
Department of Community Health and GAMMIS standards
- Complete required visit notes, supervisory reports, and reassessments
- Participate in internal audits and ensure readiness for external audits
Coordination of Care
- Communicate with physicians, case managers, and interdisciplinary teams
- Report significant changes in condition to appropriate stakeholders
- Assist with transitions of care, hospital discharges, and service adjustments
Quality Assurance
- Support Atlas ProCare’s four-eyes review system for clinical documentation
- Identify gaps in care and implement corrective action plans
- Participate in performance improvement initiatives
Minimum Qualifications
- Active Georgia RN license in good standing
- Minimum 2 years of clinical nursing experience (home health, geriatrics, or case
management preferred)
- Experience with Medicaid waiver programs or long-term care preferred
- Strong assessment, documentation, and supervisory skills
Scope of Authority
- Authorized to perform clinical assessments and create care plans
- Supervises LPNs (if applicable) and caregivers
- Responsible for clinical compliance and escalation of medical concerns
Work Environment
- Field-based (member homes) with administrative duties (remote or office)
- Travel within the Atlanta service area
Job Type: Full-time
Pay: $45.00 - $50.00 per hour
