Schedule

Flexible schedule. Position includes participation in a rotating on-call schedule, including weekends and holidays.

Position Summary

Roanoke Resource is seeking a dedicated and compassionate ACT Generalist to join our Assertive Community Treatment (ACT) Team. The ACT Generalist provides community-based mental health services to adults with serious mental illness (SMI) and co-occurring disorders. Working as part of a multidisciplinary team, this position delivers individualized services in clients’ homes and community settings to promote independence, recovery, stability, and community integration.

Under the supervision of the ACT Team Leader, the ACT Generalist participates in assessment, treatment planning, crisis intervention, rehabilitation services, and ongoing support. This position includes rotating on-call responsibilities, including overnights, weekends, and holidays.

Essential Duties and Responsibilities

· Provide comprehensive Assertive Community Treatment services in accordance with the ACT model.

· Deliver supportive interventions focused on skill restoration, psychiatric stability, and community integration.

· Assist individuals with psychiatric rehabilitation, housing stability, vocational goals, substance use recovery, and community support services.

· Conduct ongoing client assessments and participate in collaborative treatment planning.

· Provide outreach, engagement, advocacy, and case management services.

· Complete routine community and home visits based on individualized treatment plans.

· Assist individuals in maintaining independent living skills and achieving personal recovery goals.

· Provide crisis intervention and emergency response services, including during scheduled on-call rotations.

· Coordinate hospital pre-screening and discharge planning as needed.

· Assist with medication delivery, monitoring, and medication education in accordance with agency policies.

· Collaborate with psychiatrists, nurses, therapists, primary care providers, and community partners to ensure coordinated care.

· Maintain timely and accurate documentation in accordance with agency, Medicaid, and regulatory standards.

· Participate in team meetings, treatment reviews, and clinical consultations.

· Maintain confidentiality and uphold professional and ethical standards.

Qualifications

· Bachelor’s or Master’s degree in a human services or related field.

· QMHP, QMHP-T, QPPMH credential (or eligibility to obtain) required.

· Minimum of one year of direct clinical experience working with adults with serious mental illness and/or co-occurring disorders preferred.

· Experience providing mental health, case management, substance use, or rehabilitation services.

· Demonstrated crisis intervention and de-escalation skills.

· Ability to communicate and collaborate effectively with healthcare professionals, community providers, and support systems.

· Strong organizational, documentation, and time-management skills.

· Valid Virginia driver’s license with reliable transportation.

· Ability to travel independently to client homes, hospitals, community locations, and work sites.

· Ability to participate in rotating on-call coverage and provide emergency response when needed.

Physical Requirements

· Frequent travel throughout the community.

· Ability to enter client homes and community settings.

· Ability to lift and carry supplies, records, and medications as needed.

· Ability to work flexible hours, including evenings, weekends, holidays, and on-call shifts.

Pay: From $22.00 per hour
