Provides consumer related assessment tracking, enrollment and scheduling support.

Job Responsibilities: Provide Assessment related tracking, enrollment and scheduling support.

Primary Duties

1. Primary contact for scheduling of various assessment visits. Receive, distribute and schedule various assessments including initial assessments, updated assessments, adverse assessments and LOC referrals.

a. Perform data entry into the PIMS system and OB system as needed.

b. Communicate with Medicaid Managed Care plans (MCP), Job and Family Services (JFS), Adult Protective Services (APS), PASSPORT Waiver and Assisted Living waiver providers and Direction Home Assessors and Care Managers regarding referral status. Communicate with Hospitals, Nursing Homes, potential enrollees and community members as needed to provide guidance and education on the enrollment status or process.

2. Research inquiries regarding scheduling and enrollment related eligibility in the Ohio Benefits (OB) system.

3. Screening overflow coverage PRN.

Secondary Duties

4. Backup for Enrollment Specialists completing waiver enrollments and eligibility duties.

5. Backup for Assessment staff to process incoming and outgoing correspondence. Preparation and mailing of assessment related letters and maintenance of consumer electronic files.

Assist ADRC staff with correspondence and resources

Proof of influenza vaccination is required.
