Responsibilities:

• Develop and implement effective rehabilitation programs that cater to the unique needs of our residents

• Collaborate with the multidisciplinary team to ensure seamless care delivery and optimal outcomes

• Conduct assessments, develop plans of care, and implement rehabilitation interventions to enhance patients' functional abilities and quality of life

• Oversee and mentor the rehab therapy team, providing guidance, coaching, and support to ensure exceptional patient care

• Maintain accurate and thorough documentation of patient progress and rehabilitation plans

• Participate in quality improvement initiatives to drive excellence in patient care and outcomes

Requirements:

• Minimum 0 years of experience in a rehab setting, with experience in long-term care preferred

• Associates or Bachelor's degree in a relevant field, such as occupational therapy, physical therapy, or speech-language pathology

• Current licensure/certification as a clinician in the rehab team

• Strong leadership and communication skills, with the ability to motivate and inspire team members

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
