We are seeking a talented Assistant Housing Coordinator- Clinical Specialist to join our team of nearly 500 employees. The Assistant Housing Coordinator works on behalf of the Erie County Department of Mental Health (ECDMH) Adult Single Point of Access (A-SPOA) as the main point of contact for the community and providers who are submitting referrals to mental health residential services. In addition, the Assistant Housing Coordinator processes referrals to the SPOA, assigns referrals to open beds, co-facilitates case conferences, and serves as a support to the Housing Coordinator for administering the U.S. Housing and Urban Development Continuum of Care contracts that ECDMH holds. This includes, but is not limited to, processing performance data and taking part in site visits. The Assistant Housing Coordinator carries out these duties in an organized, efficient, and person-centered way. In this role, you will be under the direction of the Erie County Department of Mental Health but are an employee of Coordinated Care Services Inc (CCSI). The pay and benefits provided associated with the role reflect those offered by Coordinated Care Services Inc (CCSI) and not Erie County Department of Mental Health. You would not be eligible for any pay or benefits programs offered by Erie County.

CCSI is partnering with Erie County Department of Mental Health to hire an Assistant Housing Coordinator- Clinical Specialist. This position is employed by CCSI and working on behalf of Erie County Department of Mental Health.

In this role, you will

  • Process housing referrals submitted to the Adult Single Point of Access (SPOA)
  • Assist members of the community to navigate the mental health residential housing system through phone calls, emails, and case conferences
  • Facilitate, or assist in the facilitation, of meetings coordinating the mental health housing system of care
  • Assist the application process, administration and reporting responsibilities of HUD Continuum of Care grants
  • Assist with site monitoring visits of supportive housing grantees
  • Gather, organize and analyze data that helps inform Quality processes and supports grant applications
  • Facilitate case conference meetings to assist coordinating care for clients with challenging psychiatric needs
  • Process eligibility appeals for the A-SPOA housing referrals
  • Clinical support for supportive housing programs as needed
  • Other duties as assigned

What you bring

  • LMHC, LMSW, LCSW, CRC, or other qualified mental health professional and minimum of 1 year prior direct service experience in mental health supportive housing programs, or equivalent education and work experience (see CCSI's equivalency chart)

Required for this position are:

  • Valid driver’s license and ability to reliably travel throughout Erie County.
  • Satisfactory completion of all required background screenings
  • Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance. Staff working directly with protected health information /electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data, will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.
