We are seeking a dedicated Assistant Resident Care Coordinator to join our team at Chester Street Residence. In this role, you will play a vital part in delivering high-quality daily care and support to our residents, enhancing their quality of life while ensuring their health and safety.

Responsibilities

• Provide direct patient care and assistance to residents in daily activities.
• Administer medications and monitor residents' health conditions.
• Assist with long-term care plans and collaborate with healthcare professionals.
• Maintain accurate records of patient care and report any changes in condition.
• Support residents with mobility and heavy lifting as needed.
• Engage with residents socially to promote mental well-being.
