We’re seeking a mission-driven Attending Physician to deliver comprehensive, proactive medical care for residents across long-term care, subacute/post-acute, ventilator, and behavioral health units. You’ll round regularly, lead multidisciplinary care, and use decisive, evidence-based interventions to improve outcomes and reduce avoidable hospitalizations.
Provide direct, comprehensive medical care to residents in long-term, subacute/post-acute, ventilator, and behavioral health settings.
Conduct routine rounding, thorough evaluations, and timely follow-ups to ensure continuity of care and early, aggressive intervention.
Lead and actively participate in multidisciplinary care team meetings; contribute to individualized care plans.
Aggressively manage chronic and complex conditions; respond promptly to status changes and emergent situations using appropriate modalities (e.g., IV diuretics, antibiotics, fluids, TPN) and respiratory therapies.
Communicate clearly and compassionately with nursing staff, NPs/APPs, patients, families, and consulting providers.
Collaborate with facility nursing, rehabilitation teams, leadership, and assigned NP to coordinate and optimize care.
Discuss goals of care and palliative care options; support patients and families in shared decision-making.
Promote healthy aging through preventive care and proactive treatment plans.
Document accurately and promptly in the EMR; maintain strong clinical documentation habits.
Help develop and implement proactive strategies to reduce avoidable hospital transfers and readmissions.
Passion for patient-centered care in geriatrics and chronic disease management.
Strong clinical judgment, confidence, and decisive decision-making.
Excellent communication skills with patients, families, and interdisciplinary staff.
Proactive, assertive clinical management style; comfortable in fast-paced, diverse clinical environments.
Highly organized with effective time management; detail-oriented and efficient.
Independent and self-directed; reliable, punctual, and hardworking.
Comfortable discussing sensitive topics, including goals of care and palliative needs.
MD or DO from an accredited medical school; Board Certified/Board Eligible in Internal Medicine, Family Medicine, or Geriatrics.
Active, unrestricted state medical license (or eligibility to obtain) and current DEA registration.
Experience in post-acute/long-term care, ventilator care, or behavioral health preferred.
Proficiency with EMR systems and clinical documentation.
Demonstrated ability to collaborate effectively with NPs/APPs, nursing, rehabilitation, and administrative staff.
Current BLS/ACLS (or willingness to obtain, as required by facility).
